机器人辅助肝癌根治术治疗乙肝相关性肝癌的疗效分析OA
Efficacy of robot-assisted radical hepatectomy in the treatment of hepatitis B virus-related hepatocellular carcinoma
目的:分析机器人辅助肝癌根治术在乙肝相关性肝癌治疗中的临床效果.方法:选取2024年1月—2025年6月于中国医科大学附属盛京医院收治的120例乙型肝炎相关性肝癌手术患者,根据随机数字表法分为对照组(行开腹肝癌根治术)与观察组(行机器人辅助肝癌根治术),每组各60例.比较两组患者手术指标、术后康复指标、术后疼痛情况及围手术期肝功能和炎症介质变化情况.结果:观察组手术总时间长于对照组,肝门阻断总时间、切缘距离、术后首次下床活动时间、恢复排气时间、恢复排便时间及总住院时间均短于对照组,术中出血量少于对照组(P<0.05).观察组术后12 h、24 h、48 h、72 h的疼痛视觉模拟评分法评分均低于对照组(P<0.05).术后3 d,观察组AST、ALT、TBil、IL-6、CRP、ALB、TNF-α水平的变化幅度均小于对照组(P<0.05).结论:相较于开腹肝癌根治术,机器人辅助肝癌根治术治疗乙肝相关性肝癌优势更显著,可提高手术整体效率,减小手术创伤,充分切除肿瘤,从而促进患者术后康复,减轻肝功能损伤及机体炎症反应.
Objective:To analyze the clinical efficacy of robot-assisted radical hepatectomy in the treatment of hepatitis B virus(HBV)-related hepatocellular carcinoma(HCC).Methods:120 patients with HBV-related HCC who underwent radical hepatectomy at Shengjing Hospital of China Medical University from January 2024 to June 2025 were enrolled.They were randomly divided into the control group(n=60,undergoing open radical hepatectomy)and the observation group(n=60,undergoing robot-assisted radical hepatectomy)using a random number table.Surgical indicators,postoperative recovery indicators,pain levels,and perioperative changes in liver function and inflammatory mediators were compared between the two groups.Results:The total operative time was longer in the observation group than that in the control group,while the total portal clamping time,resection margin distance,time to first ambulation,time to first flatus,time to first defecation,and total length of hospital stay were significantly shorter in the observation group(P<0.05).Intraoperative blood loss was significantly lower in the observation group than that in the control group(P<0.05).Visual analog scale(VAS)scores at 12 h,24 h,48 h,and 72 h postoperatively were all significantly lower in the observation group than those in the control group(P<0.05).3 days after surgery,the magnitudes of change in ALT,AST,TBil,IL-6,CRP,ALB,and TNF-α were significantly smaller in the observation group than those in the control group(P<0.05).Conclusion:Compared with open radical hepatectomy,robot-assisted radical hepatectomy has greater advantages in treating HBV-related HCC.It can enhance overall surgical efficiency,reduce surgical trauma,ensure complete tumor resection,thereby promoting postoperative recovery,reducing liver function impairment and decreasing systemic inflammatory response.
