机器人辅助心脏外科手术的安全性及中期疗效:一项单中心队列研究OA
Safety and mid-term outcomes of robot-assisted cardiac surgery:a single-center cohort study
目的:总结机器人辅助心脏外科手术的临床疗效及治疗经验.方法:收集2020年7月—2022年9月于西北大学附属西安国际医学中心医院心脏血管外科接受达芬奇Xi手术系统辅助心脏外科手术的31例患者临床资料,分析围手术期指标及术后随访结果.结果:术中中转开胸3例,二次开胸探查2例.术后1例因严重肺损伤行体外膜肺氧合短期辅助治疗,7例因肺损伤致呼吸机辅助时间延长(>24 h),2例因肾衰竭行持续性肾替代治疗.11例出现手术相关肝损伤,无手术相关脑卒中发生.1例因低心排血量综合征导致术后早期死亡,其余30例患者康复出院.术后最长随访61个月,术后1年及5年生存率均为93.55%.结论:机器人辅助心脏外科手术创伤小、安全可靠、中期预后良好,但其学习曲线较为陡峭,初期主动脉阻断时间、体外循环转流时间及手术时间较长,需积累一定手术量后方可逐步缩短.
Objective:To summarize the clinical efficacy and treatment experience of robot-assisted cardiac surgery.Methods:Clinical data of 31 patients who underwent robot-assisted cardiac surgery using the Da Vinci Xi surgical system at the Department of Cardiovascular Surgery,Xi'an International Medical Center Hospital Affiliated to Northwest University from July 2020 to September 2022 were collected.Perioperative indicators and postoperative follow-up outcomes were analyzed.Results:There were three cases of intraoperative conversion to thoracotomy and two cases of secondary thoracic exploration.Postoperatively,one patient received extracorporeal membrane oxygenation(ECMO)due to severe pulmonary injury,seven patients experienced prolonged mechanical ventilation(>24 h)due to lung injury,two patients required continuous renal replacement therapy due to renal failure,and 11 patients developed surgery-related liver injury.No surgery-related stroke occurred.One patient died early postoperatively due to low cardiac output syndrome,and the remaining 30 patients were successfully discharged.The maximum follow-up duration was 61 months,and the 1-year and 5-year survival rates were both 93.55%.Conclusion:Robot-assisted cardiac surgery is characterized by minimal invasiveness,safety,reliability,and favorable medium-term prognosis.However,it has a steep learning curve.Prolonged aortic cross-clamp time,cardiopulmonary bypass time,and total operative time can be observed in the initial phase,which tend to decline progressively with accumulated surgical experience.
