5G远程超声机器人辅助下精准经皮肾穿刺治疗复杂性肾结石的临床研究OA
Clinical study on precise percutaneous nephrolithotomy for the treatment of complex renal calculi assisted by 5G remote ultrasound robot
目的:探讨5G 远程超声机器人辅助下精准经皮肾穿刺技术治疗复杂性肾结石的有效性与安全性.方法:回顾分析2023 年10 月至2025 年3 月收治的64 例复杂性肾结石患者的临床资料.手术借助5G 远程超声机器人辅助完成精准经皮肾穿刺以建立手术操作通道,行钬激光碎石取石.总结分析手术时间、穿刺次数、通道建立时间、术中出血量、结石清除率、术后并发症及术中5G 网络运行情况.结果:64 例手术均顺利完成,手术时间平均(86.3±15.8)min,穿刺次数平均(1.9±0.5)次;通道建立时间(12.0±5.5)min,术中血红蛋白下降(11.4±2.1)g/L;术中网络最大延迟72.5 ms,数据丢包率3.1%,无网络中断及数据泄露等事件发生.术后第1 天、1 个月结石清除率分别为 84.4%与 90.6%.术后发生出血 6 例(9.4%),其中轻度出血4 例,保守治疗后出血停止,中度出血2 例,行选择性肾动脉栓塞后出血得到有效控制;感染3 例(4.7%),予以敏感抗生素抗感染治疗后体温恢复正常;未发生周围脏器损伤、尿瘘等严重并发症.结论:5G 远程超声机器人辅助下精准经皮肾穿刺技术为基层医院治疗复杂性肾结石提供了安全、有效的新方法,实现了优质医疗资源下沉,具有临床推广价值.
Objective:To explore the efficacy and safety of 5G remote ultrasound robot-assisted precise percutaneous nephro-lithotomy in the treatment of complex renal calculi.Methods:A retrospective analysis was performed on the clinical data of 64 patients with complex renal calculi admitted from Oct.2023 to Mar.2025.All surgeries were performed with the assistance of 5G remote ultra-sound robot to complete precise percutaneous nephrostomy for establishing surgical operation channels,followed by holmium laser litho-tripsy and calculi extraction.The operation time,number of punctures,channel establishment time,intraoperative blood loss,calculi clearance rate,postoperative complications,and intraoperative 5G network operation status were summarized and analyzed.Results:All 64 surgeries were successfully completed.The average operation time was(86.3±15.8)min,with an average of(1.9±0.5)punctures per case.The channel establishment time was(12.0±5.5)min,and the intraoperative hemoglobin decrease was(11.4±2.1)g/L.Du-ring the operation,the maximum network delay was72.5 ms,the data packet loss rate was3.1%,and no events such as network inter-ruption or data leakage occurred.The calculi clearance rates on the first postoperative day and one month postoperatively were 84.4%and 90.6%,respectively.Postoperative bleeding occurred in 6 cases(9.4%),including 4 cases of mild bleeding which stopped after conservative treatment,and 2 cases of moderate bleeding which were effectively controlled by selective renal artery embolization;infec-tion occurred in 3 cases(4.7%),and the body temperature returned to normal after anti-infection treatment with sensitive antibiotics.No severe complications such as adjacent organ injury or urinary fistula occurred.Conclusions:The 5G remote ultrasound robot-assisted precise percutaneous nephrolithotomy provides a safe and effective new method for primary-level hospitals in the treatment of complex renal calculi,realizes the sinking of high-quality medical resources,and has clinical promotion value.
