Analysis of Lujiaojiao(Cervi Cornus Colla)in Treatment of Patients with Knee Osteoarthritis of Wind-Cold-Damp Paralysis and Deficiency of Liver and Kidney Syndrome

目的 观察鹿骨胶治疗膝骨性关节炎风寒湿痹兼肝肾亏虚证患者的临床运用效果.方法 选取2024年3月-2024年6月在山东第一医科大学附属人民医院骨科就诊的94例膝骨性关节炎患者为研究对象,采用简单随机法分为两组.伊索佳组47例(脱落2例,完成45例资料收集)采用硫酸氨基葡萄糖胶囊(商品名:伊索佳),鹿骨胶组47例(脱落2例,完成45例资料收集)采用鹿骨胶联合伊索佳.统计两组疗效和安全性,比较两组关节液基质金属蛋白酶-3(Matrix metalloproteinase-3,MMP-3)、金属蛋白酶抑制剂-1(Metalloproteinase inhibitor-1,TIMP-1)、数字评定量表(Numeric rating scale,NRS)评分、膝关节评分、超声检测参数及血清骨代谢水平.结果 鹿骨胶组总有效率为91.11%(41/45),高于伊索佳组的75.56%(34/45),差异有统计学意义(P<0.05).两组治疗前膝关节评分、超声检测参数、NRS评分、膝骨关节炎主要症状体征评分比较,差异无统计学意义(P>0.05).两组治疗后膝关节评分较前升高,鹿骨胶组更高(P<0.05).两组治疗后膝骨关节炎主要症状体征评分、滑膜厚度、NRS评分、关节积液深度均较前降低,鹿骨胶组更低(P<0.05).两组治疗前关节液MMP-3、TIMP-1及血清骨代谢比较,差异无统计学意义(P>0.05).两组治疗后关节液TIMP-1及血清骨钙素(Osteocalcin,BGP)、骨特异性碱性磷酸酶(Bone specific alkaline phosphatase,BALP)较前升高,鹿骨胶组更高(P<0.05).两组治疗后关节液MMP-3、MMP-3/TIMP-1及血清抗酒石酸盐酸性磷酸酶异构体 5b(Tartrate resistant hydrochloride phosphatase isomer 5b,TRACP-5b)、Ⅰ 型胶原 C 端肽(C-terminal peptide of type Ⅰ collagen,CTX-Ⅰ)均较前降低,鹿骨胶组更低(P<0.05).鹿骨胶组不良反应发生率11.11%(5/45),伊索佳组不良反应发生率为8.89%(4/45),差异无统计学意义(P>0.05).结论 鹿骨胶治疗膝骨性关节炎可维持关节液MMP-3/TIMP-1动态平衡,改善骨代谢,提高疗效.

Objective To observe the effect of clinical use of Lujiaojiao(Cervi Cornus Colla)in the treatment of patients with knee osteoarthritis of wind-cold-damp paralysis and deficiency of liver and kidney syndrome.Methods Ninety-four patients with knee osteoarthritis attending the Department of Orthopaedics of People's Hospital Affiliated to Shandong First Medical Univer-sity from March 2024 to June 2024 were selected and divided into two groups by simple randomisation method.In the Isogia group,47 cases(2 cases were dislodged and 45 cases were completed the trail)were treated with Glucosamine Sulfate Capsules(trade name:Isogia),and in the Lujiaojiao(Cervi Cornus Colla)group,47 cases(2 cases were dislodged and 45 cases were com-pleted the trail)were treated with Lujiaojiao(Cervi Cornus Colla)combined with Isogia.The efficacy and safety of the two groups were counted,and the joint fluid matrix metalloproteinase-3(MMP-3),metalloproteinase inhibitor-1(TIMP-1),numerical rating scale(NRS)score,knee score,ultrasound parameters and serum bone metabolism level were compared between the two groups.Results The total effective rate of the Lujiaojiao(Cervi Cornus Colla)group was 91.11%(41/45).The total effective rate of the Isojia group was 75.56%(34/45).The total effective rate of the Lujiaojiao(Cervi Cornus Colla)group was higher than that of the Isojia group,and there was a difference(P<0.05).The comparison of knee joint score,ultrasound detection pa-rameters,NRS score and score of main symptoms and signs of knee osteoarthritis before treatment between the two groups showed no difference(P>0.05).The knee joint scores were higher in both groups after treatment and the scores were higher in the Deer Bone Glue group than those of the Isojia group(P<0.05).Knee osteoarthritis major symptoms and signs score,synovial thick-ness,NRS score and joint fluid depth after treatment in both groups were lower than those before treatment,and the scores were lower in the Lujiaojiao(Cervi Cornus Colla)group than those of the Isojia group(P<0.05).The comparison of joint fluid MMP-3,TIMP-1 and serum bone metabolism before treatment between the two groups showed no difference(P>0.05).The levels of joint fluid TIMP-1 and serum osteocalcin(BGP)and bone-specific alkaline phosphatase(BALP)were elevated after treatment in both groups compared with those before treatment,and the levels were higher in the Deer Bone Glue group than those of the Isojia group(P<0.05).The levels of joint fluid MMP-3,MMP-3/TIMP-1 and serum anti-tartrate acid phosphatase isoform 5b(TRACP-5b)and collagen type Ⅰ C-terminal peptide(CTX-Ⅰ)were lower in both groups after treatment than those before treatment,and the levels were lower in the Lujiaojiao(Cervi Cornus Colla)group than those of the Isojia group(P<0.05).A total of 5 cases of adverse reactions occurred in the Lujiaojiao(Cervi Cornus Colla)group,with an incidence rate of 11.11%(5/45);4 cases occurred in the Isojia group,with an incidence rate of 8.89%(4/45).There was no difference in the cumulative incidence of adverse reactions between the two groups(P>0.05).Conclusion Lujiaojiao(Cervi Cornus Colla)can maintain the dynamic balance of MMP-3/TIMP-1 in synovial fluid,improve bone metabolism and enhance the efficacy in the treatment for knee osteoarthritis.
