双肾汤对紫癜性肾炎患儿炎症因子、免疫功能及复发的影响OA
Effects of Shuangshen Decoction(双肾汤)on Inflammatory Factors,Immune Function and Recurrence in Children with Purpura Nephritis
目的 探讨双肾汤对紫癜性肾炎患儿炎症因子、免疫功能及复发的影响.方法 选取100例紫癜性肾炎患儿,随机分为对照组与观察组,每组50例,对照组给予糖皮质激素治疗,观察组同时服用双肾汤,观察治疗前及治疗12周时中医证候积分、血清炎症因子及免疫功能变化,随访12个月内感染复发情况.结果 两组治疗前中医证候积分比较差异无统计学意义(P＞0.05);两组治疗后中医证候积分小于治疗前(P＜0.05);同时治疗后观察组中医证候积分小于对照组(P＜0.05).两组治疗前血清肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素(interleukin,IL)-6、IL-10水平比较差异无统计学意义(P＞0.05);两组治疗后均低于治疗前(P＜0.05);同时治疗后观察组水平低于对照组(P＜0.05);两组治疗前血清CD4+、CD8+、免疫球蛋白(immunoglobulin,Ig)A、IgG、IgM水平比较差异无统计学意义(P＞0.05);观察组治疗后CD4+、IgA、IgG水平高于治疗前,同时高于治疗后对照组同期,差异有统计学意义(P＜0.05);1年内观察组复发率及感染率分别为22.00％(11/50)、34.00％(17/50),低于对照组[44.00％(22/50)、62.00％(31/50)],复发次数及感染次数低于对照组,差异有统计学意义(P＜0.05);观察组疗效高于对照组(P＜0.05).结论 双肾汤治疗儿童紫癜性肾炎可有助于减轻症状,改善免疫功能及炎症反应,降低感染及复发率.
Objective To investigate the effects of Shuangshen Decoction(双肾汤)on inflammatory factors,immune function and recurrence in children with purpura nephritis.Methods A hundred cases with purpura nephritis were randomly divided into the control group and the observation group,50 cases in each group.Both groups were given glucocorticoid therapy,and the ob-servation group was given Shuangshen Decoction simultaneously.The changes of syndrome scores,serum inflammatory factors and immune function were observed before and 12 weeks after treatment,and the recurrence of infection was followed up within 12 months.Results There was no statistically significant difference in the scores of traditional Chinese medicine symptoms between the two groups before treatment(P＞0.05).The scores decreased at after treatment(P＜0.05)and the decline degree in the ob-servation group was greater than that in the control group(P＜0.05).There was no statistically significant difference in the ser-um tumor necrosis factor-α(TNF-α),interleukin(IL)-6 or IL-10 between the two groups before treatment(P＞0.05).There were decreased after treatment(P＜0.05)and the decline degree in the observation group was greater than that in the con-trol group(P＜0.05).There was no statistically significant difference in the CD4+,CD8+,IgA,IgG or IgM between the two groups before treatment(P＞0.05).The levels of CD4+,IgA and IgG in the observation group were higher after treatment than those before treatment(P＜0.05),and it was higher than that of the control group after treatment(P＜0.05).The recurrence rate and infection rate of the observation group were 22.00％(11/50)and 34.00％(17/50),which were lower than those of the control group[44.00％(22/50)and 62.00％(31/50)](P＜0.05).The number of recurrence and infection in the observation group was lower than that in the control group(P＜0.05),and the therapeutic effect in the observation group was higher than that in the control group(P＜0.05).Conclusion Shuangshen Decoction in the treatment of children with purpura nephritis can help to alleviate symptoms,improve immune function and inflammatory response,and reduce infection and recurrence rate.
