偏颌患者(牙合)平面横向倾斜度和Spee曲线改变对颞下颌关节的生物力学分析OA
Biomechanical analysis of the transverse occlusal plane inclination and spee curve alteration on the temporomandibular joint in patients with mandibular deviation
目的 探讨偏颌患者(牙合)平面横向偏斜矫正及Spee曲线整平对颞下颌关节(TMJ)中髁突应力分布的影响,为临床矫治策略提供生物力学依据.方法 基于偏颌畸形患者的锥形束CT(CBCT)数据,建立下颌骨、牙齿及颞下颌关节的三维有限元模型.设置4种工况:工况0(原始偏颌状态)、工况1(偏斜侧牙尖增高)、工况2(非偏斜侧牙尖降低)、工况3(Spee曲线整平).分析双侧髁突在牙尖交错位咬合载荷下的等效应力、最大主应力和最小主应力分布.结果 基准模型(工况0)显示非偏斜侧髁突应力显著高于偏斜侧,工况1和工况2虽改变了应力数值,但未改变应力集中模式,工况3(Spee曲线整平)在适度增大非偏斜侧应力的同时,显著降低了偏斜侧应力,并使双侧应力分布更为分散,有效缓解了局部应力集中.结论 相较于单纯横向矫正骀平面,整平Spee曲线能更有效地优化偏颌患者TMJ的应力分布,改善生物力学环境.提示在偏颌畸形矫治中,应重视垂直向控制,尤其是对过陡Spee曲线的处理.
Objective To investigate the influence of transverse occlusal plane leveling and Spee curve flattening on stress distribution in the temporomandibular joint(TMJ)condyles in patients with mandibular laterognathism,providing a biomechanical basis for clinical treatment strategies.Methods Based on Cone-beam CT(Cone Beam Computed Tomography,CBCT)data from a patient with mandibular laterognathism,three-dimensional finite ele-ment models of the mandible,teeth,and temporomandibular joints were established.Four working conditions were set:Condition 0(original laterognathic state),Condition 1(increasing cusp height on the deviated side),Condition 2(decreasing cusp height on the non-deviated side),and Condition 3(Spee curve flattening).The distribution of von Mises stress,maximum principal stress,and minimum principal stress in bilateral condyles under intercuspation loading was analyzed.Results The baseline model(Condition 0)showed significantly high-er stress in the condyle on the non-deviated side compared to the deviated side.Although Conditions 1 and 2 al-tered the stress values,they did not change the stress concentration pattern.Condition 3(Spee curve flatten-ing),while moderately increasing stress on the non-deviated side,significantly reduced stress on the deviated side and resulted in a more dispersed stress distribution on both sides,effectively alleviating localized stress con-centration.Conclusion Compared with simple transverse correction of the occlusal plane,flattening the Spee curve can more effectively optimize stress distribution in the TMJ of patients with mandibular laterognathism,im-proving the biomechanical environment.This suggests that vertical control,especially the management of an ex-cessively steep Spee curve,should be emphasized in the treatment of mandibular laterognathism.
