基于氧化应激探究中医药治疗男性不育症的研究进展OA
Research progress on traditional Chinese medicine treatment of oxidative stress-induced male infertility
男性不育症是指处于生育年龄的夫妇在未实施避孕措施的前提下规律性行为持续 12个月以上,男性因素导致配偶未能正常受孕的疾病.近年来,随着社会经济的不断发展与生活方式的转变,男性不育症发病率在全球范围内呈逐年上升趋势.其致病机制极为复杂,涉及遗传异常、内分泌紊乱、生殖道感染、精索静脉曲张、免疫因素、不良生活方式及环境暴露等多种病因.近年来的研究进一步揭示,氧化应激是贯穿多种病因的核心病理环节之一.西医学主要采用药物、手术及辅助生殖技术进行治疗,虽取得一定成效,但仍存在局限性,如部分病因治疗困难、药物不良反应、手术风险、辅助生殖技术成本高昂及伦理争议等.中医药凭借其整体观念、辨证论治的独特理论体系展现出多成分、多靶点、多途径综合干预的系统性优势,在改善男性不育症患者精液质量、提高自然妊娠率方面积累了丰富的临床经验.大量研究表明,中医药治疗男性不育症的机制与增强抗氧化酶的活性,抑制脂质过氧化,加快氧化应激产物的清除密切相关.为进一步梳理该领域的研究进展,文章从中药单体、中药复方、中成药、针灸、综合疗法等维度全面归纳与阐述近年来中医药干预男性不育症抗氧化应激的试验与临床研究进展,旨在为后续的研究与临床治疗提供参考.
Male infertility(MI)refers to the failure of a couple to achieve a successful pregnancy after at least 12 months of regular,unprotected sexual intercourse,due to male factors.In recent years,alongside socio-economic development and shifts in lifestyle patterns,the global incidence of MI has shown a concerning upward trend.Its pathogenic mechanisms are highly complex,involving multiple etiologies such as genetic abnormalities,endocrine disorders,reproductive tract infections,varicocele,immunological factors,adverse lifestyle habits,and environmental exposures. Emerging research further indicates that oxidative stress serves as a core pathological link underlying many of these diverse causes. Contemporary medicine primarily employs pharmacological,surgical,and assisted reproductive technologies (ART) to treat MI. While these approaches have achieved certain success,they present limitations,including difficulty in addressing some root causes,drug side effects,surgical risks,the high cost of ART,and associated ethical controversies. In this context,traditional Chinese medicine (TCM),guided by its unique theoretical framework emphasizing a holistic perspective and syndrome differentiation,exhibits systematic advantages through multi-component,multi-target,and multi-pathway comprehensive interventions.TCM has accumulated substantial clinical experience in improving semen parameters and enhancing natural pregnancy rates in MI patients.A significant body of evidence suggests that the therapeutic mechanisms of TCM for MI are closely related to its potent antioxidant effects,primarily involving the enhancement of antioxidant enzyme activity,inhibition of lipid peroxidation,and promotion of oxidative stress product clearance. To better summarize advances in this field,this article comprehensively reviews and elaborates on recent experimental and clinical research progress regarding the antioxidative stress effects of TCM interventions for MI. The review is organized around several key aspects:active herbal monomers,compound herbal formulations,patented Chinese medicines,acupuncture,and integrated therapeutic approaches. The aim is to provide a reference for subsequent research and clinical practice.
