A review on the treatment of sepsis in traditional Chinese medicine based on miRNAs and related signaling pathways

脓毒症是由细菌等病原微生物侵入机体引起的全身炎性反应综合征,其发病机制复杂,与机体多系统、多器官病理生理改变密切相关.微小核糖核酸(micro ribonucleic acids,miRNAs)作为一种重要的调控因子,参与细胞的生物学过程,如增殖、凋亡、分化及存活,对生物体基因调控至关重要,且对多种疾病具有预测价值.研究证实,miRNAs 可以介导炎性反应、免疫调控及氧化应激的过程,且与酪氨酸激酶(janus kinase,JAK)-信号转导子和转录激活子(signal transducer and activator of transcription,STAT)、核转录因子-κB(nuclear factor kappa B,NF-κB)、磷脂酰肌醇 3 激酶(phosphatidylinositol 3-kinase,PI3K)/蛋白激酶 B(Akt)等信号通路密切相关,在脓毒症的发生与发展中起着关键作用.目前,脓毒症一线治疗药物仍以抗生素为主.然而,抗生素的长期使用所产生的一系列问题也逐渐显露.近年来,随着科学技术的不断发展,中医药在脓毒症治疗领域的研究取得了重要进展.中药单体(如连翘苷、人参皂苷、黄芩素等)、复方(如化瘀方、升降散、血必净注射液等)及中医外治可通过上调或下调特定 miRNAs 表达,调节炎症因子肿瘤坏死因子(tumor necrosis factor,TNF-α)、白细胞介素(interleukin,IL)-6、IL-1β 等释放,改善氧化应激指标超氧化物歧化酶(superoxide dismutase,SOD)、谷胱甘肽活性及丙二醛(malondialdehyde,MDA)含量,抑制靶器官细胞异常凋亡,保护心、肺、肾等重要脏器功能.文章系统综述 miRNAs 与脓毒症相关信号通路的关联及中医药调控 miRNAs 的作用机制,总结中药单体、中药复方/制剂及中医外治的干预效果,旨在为中医药治疗脓毒症开辟新的途径,提高患者的生存率和生活质量.

Sepsis is a systemic inflammatory response syndrome(SIRS)caused by the invasion of the body by pathogenic microorganisms such as bacteria.Its pathogenesis is complex and closely associated with the pathophysiological changes of multiple systems and organs in the body.As important regulatory factors,microRNAs(miRNAs)participate in cellular biological processes including proliferation,apoptosis,differentiation,and survival,which are crucial for gene regulation in organisms and possess predictive value for various diseases.Studies have confirmed that miRNAs can mediate inflammatory response,immune regulation,and oxidative stress processes,and are closely related to signaling pathways such as JAK-STAT,NF-κB,and PI3K/Akt,playing a key role in the occurrence and development of sepsis.Currently,antibiotics remain the dominant first-line therapeutic drugs for sepsis.However,a series of problems arising from the long-term use of antibiotics have gradually emerged.In recent years,with the continuous advancement of science and technology,significant progress has been made in the research of traditional Chinese medicine(TCM)in the field of sepsis treatment.TCM monomers(e.g.,phillyrin,ginsenoside,baicalein),TCM compound prescriptions[e.g.,Huayu Fang(化瘀方),Shengjiang San(升降散),Xuebijing injection(血必净注射液)],and TCM external therapies can regulate the release of inflammatory factors such as TNF-α,IL-6,and IL-1β,improve oxidative stress indicators including the activities of SOD and GSH as well as the content of MDA,inhibit abnormal apoptosis of target organ cells,and protect the functions of vital organs such as the heart,lungs,and kidneys by upregulating or downregulating the expression of specific miRNAs.This article systematically reviews the association between miRNAs and sepsis-related signaling pathways as well as the mechanisms by which TCM regulates miRNAs,and summarizes the intervention effects of TCM monomers,TCM compounds/preparations,and TCM external therapies.It aims to open up new avenues for TCM treatment of sepsis and provide a useful reference for improving the survival rate and quality of life of patients.
