Chronic constipation is the most common gastrointestinal disease in clinical practice.This article summarizes Professor CHU Quan gen's clinical experience in differentiating and treating chronic constipation based on the theory of"treating the stopped by stopping"in traditional Chinese medicine(TCM).The pathogenesis can be summarized into three aspects:the root cause is the deficiency of the spleen and stomach in the middle Jiao(焦).The fundamental cause is the dysfunction of Qi(气)movement and the failure of its ascending and descending;and the progression is due to deficiency induced excess.Based on this,the treatment is guided by the"treating the stopped by stopping"theory,aiming to treat the root cause and open the blockage by replenishment.Four therapeutic approaches are proposed to replenish the middle Jiao:warming Yang(阳),replenishing Qi,nourishing Yin(阴),and enriching blood.Additionally,the treatment aims to regulate Qi movement and restore the normal ascending and descending functions to ensure smooth circulation of Qi and blood.For cases where deficiency induced excess,a combination of purgation and replenishment is used to remove stagnation and dredge the collaterals.The ultimate goal is to restore the intestinal function,and the symptoms of constipation will subsequently disappear.One clinical case is attached at the end of the article for illustrative example.
