抑郁与卒中共病的异病同治机制:聚焦痰瘀-炎症轴的中西医整合研究OA
Mechanism of treating different diseases with the same therapy in depression and stroke comorbidity:an integrated Chinese and Western medicine perspective on the phlegm stasis-inflammation axis
抑郁与卒中是常见的神经系统疾病,二者共病率高,病理上相互影响,已成为当前脑病研究领域的焦点与难点.西医学虽在抗炎、神经保护等方面取得了一定进展,但对于抑郁与卒中共病这一复杂病理网络的整体调控仍存在局限性,尤其是单一靶点药物的应答率低、靶向性不足等问题突出.因此,从中西医结合的整合医学视角出发,探索抑郁与卒中共病的生物学基础与干预策略,具有重要的理论创新价值和临床实践意义.基于"中西医整合"思路,深入挖掘"炎症轴"与"痰瘀"理论的相关性,提出"痰瘀-炎症轴"是二者共同的核心病机.中医理论认为,痰瘀互结、壅滞窍络,既可扰及神明而致郁,亦可阻滞脑脉而致卒中,这与神经免疫炎性反应中细胞因子释放、血管内皮损伤等过程存在高度对应关系.由此,"痰瘀-炎症轴"不仅为理解两种疾病的共同病理基础提供了理论框架,更成为"异病同治"策略的关键切入点.同时结合临床实践,探讨中药靶点治疗与中医外治干预抑郁和卒中的疗效.文章汇通现代神经免疫机制与中医痰瘀理论,不仅为抑郁与卒中的防治提供了"病证结合、异病同治"的新思路与方法,也体现了中医整体观念与辨证论治在复杂疾病治疗中的整合价值,为跨病种的精准治疗模式提供了突破性的学术思路.
Depression and stroke are common neurological disorders with a high comorbidity rate and pathologically interrelated mechanisms,making them a focal point and a major challenge in contemporary brain disease research.While modern medicine has achieved certain advances in anti-inflammatory and neuroprotective therapies,comprehensive regulation of the interrelated pathological network underlying depression-stroke comorbidity remains clinically limited.This is particularly evident in the low response rates and insufficient target engagement efficacy of single-target drugs.Therefore,exploring the biological basis and interventional strategies for depression-stroke comorbidity from an integrated medicine perspective combining Chinese and Western medicine perspective holds significant theoretical innovation value and clinical practical significance.This paper,based on the concept of"integrated of Chinese-Western concept",delves into the correlation between the"inflammation axis"and the"phlegm-stasis"theory,proposing that the"phlegm-stasis-inflammation axis"represents the shared core pathogenesis of both conditions.Traditional Chinese medicine theory posits that the mutual intertwining of phlegm and blood stasis obstructs the orifices and collaterals,potentially disturbing mental clarity to induce depression while also impeding cerebral vessels to cause stroke.This closely corresponds to processes in neuroimmune and inflammatory responses,such as cytokine release and vascular endothelial injury.Thus,the"phlegm-stasis-inflammation axis"not only provides a theoretical framework for understanding the shared pathological basis of these two diseases but also serves as a key entry point for the"treating different diseases with the same therapy"strategy.Concurrently,clinical practice explores the efficacy of targeted therapy with Chinese herbal medicine and TCM external interventions for depression and stroke.This paper integrates modern neuroimmunological mechanisms with TCM phlegm-blood stasis theory,offering novel approaches for the prevention and treatment of depression and stroke through"combining disease patterns with syndrome differentiation"and"treating different diseases with the same therapy".It demonstrates the integrative value of TCM's holistic perspective and syndrome differentiation in managing complex diseases,providing a breakthrough academic framework for cross-disease precision treatment models.
