基于古今医案数据挖掘的肾虚型喘证用药规律探析OA

目的:对古今中医药治疗肾虚型喘证用药进行研究分析,探讨古今中医治疗肾虚型喘证用药规律及异同,为喘证用药研究提供新的思路与理论依据.方法:检索知网、万方、《中华医典》、古今医案云平台 4 个数据库中肾虚型喘证相关文献,使用 Excel 2019 建立治疗肾虚型喘证的方药数据库,通过 SPSS Modeler 18.0、Cytoscape 3.10、Origin 2024 对组方用药进行药物频数、性味归经、功效、关联规则、复杂网络和系统聚类分析,然后运用中医学理论对统计结果进行分析讨论.结果:总计筛选出符合纳入标准的中药处方 287 首,涉及中药 297 种,累计用药 3 196 次,挖掘出使用频数≥20 的中药 42 味.对中药性味归经进行分析可知,药物四气以温性频数最高,五味以甘味使用频数最高,归经以肺经、肝经、肾经为最.设置最小支持度为 10%,置信度≥80%,得到关联组合 25 项.复杂网络生成的核心组方为茯苓、山茱萸、山药、熟地黄、五味子.通过系统聚类得到 5组药物聚类组合.结论:肾虚型喘证治疗以补肾纳气为核心,兼顾健脾祛湿、敛肺化痰,用药凸显滋阴温阳并用、标本同治的特点,充分体现"脾肾同治"思路.数据挖掘得到的药物组合对临床用药有较好的参考价值.本研究揭示了喘证用药规律,印证"肾主纳气"理论的价值,为临床优化处方、提升诊疗效果提供数据支撑.

Objective:To analyze the medication characteristics and rules of traditional Chinese medicines(TCM)used in treating asthma with kidney deficiency syndrome in ancient and modern Chinese medical cases,explore the distribution similarities and differences of drugs used for this syndrome,and provide new ideas and theoretical basis for the application research.Methods:By searching the related literature of asthma with kidney deficiency in CNKI,Wanfang Data,Chinese Medical Canon,and Ancient and Modern Medical Cases Cloud Platform,the database of prescriptions and herbs for this syndrome was established using Excel 2019,and the herb frequency,four natures and five flavor,meridian tropism,efficacy,association rules,complex network and system clustering were analyzed via SPSS Modeler 18.0,Cytoscape 3.10,and Origin 2024.Results:A total of 287 Chinese medicine prescriptions meeting the inclusion criteria were screened out,involving 297 kinds of Chinese medicinal herbs with a cumulative medication frequency of 3 196 times.A total of 42 Chinese medicinal herbs with a usage frequency of≥20 times were identified.Analysis of the nature,flavor,and meridian tropism of the Chinese medicinal herbs showed that in term of four natures,warm-natured herbs had the highest frequency;in term of five flavors,sweet-flavored herbs were the most frequently used;and in terms of meridian tropism,the herbs mainly acted on the lung meridian,liver meridian,and kidney meridian.With the minimum support set at 10%and confidence level≥80%,25 association combinations were obtained.The core formula generated via complex network analysis consisted of Fuling(Poria cocos),Shanzhuyu(Fructus Corni),Shanyao(Rhizoma Dioscoreae),Shudihuang(Radix Rehmanniae Preparata),and Wuweizi(Fructus Schisandrae Chinensis).In addition,5 herb clustering combinations were identified through hierarchical clustering analysis.Conclusion:The treatment of asthma with kidney deficiency syndrome is centered on tonifying kidney and reinforcing Qi(气),giving consideration to invigorating spleen and eliminating dampness,astringing lung and resolving phlegm,and the medication highlights the characteristics of nourishing Yin(阴)and warming Yang(阳),treating both the symptoms and root causes,which fully embodies the idea of"treating spleen and kidney simultaneously".The prescription combination obtained by data mining has good reference value for clinical medication.This study reveals the law of medication for asthma with kidney deficiency syndrome,confirms the theoretical value of"kidney governing Qi"in TCM,and provides data support for clinical optimization of prescription and improvement of diagnosis and treatment effect.
