不同灸法对肺癌患者免疫功能干预效果的网状Meta分析OA
Network Meta-analysis:a comparison of the intervention effects of different moxibustion therapies on immune function in lung cancer patients
目的:运用网状 Meta 分析比较不同灸法对肺癌患者免疫功能的影响,并进行有效性排序,从循证医学角度筛选出临床最佳方案.方法:计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、Web of Science、Embase、CINAHL、Cochrane Library 中灸法对肺癌患者免疫功能影响的中、英文随机对照试验文献,检索时限为建库至 2025 年3月20日,基于Cochrane推荐的"偏倚风险评估工具RoB2.0"评估偏倚风险,采用RevMan 5.4和Stata 18进行基于贝叶斯模型的网状 Meta 分析.结果:最终纳入 17 项研究,涉及 1 472 例患者、7 种灸法,干预时长 14~105 天.针对 CD3+指标,最佳效果概率排序依次为热敏灸、麦粒灸、益气养阴汤、雷火灸、艾灸、温针灸、督灸、益气健脾方、常规治疗;针对 CD4+指标,最佳效果概率排序依次为益气养阴汤、热敏灸、督灸、温针灸、雷火灸、艾灸、麦粒灸、益气健脾方、常规治疗;针对 CD8+指标,最佳效果概率排序依次为热敏灸、督灸、益气健脾方、温针灸、麦粒灸、雷火灸、艾灸、常规治疗;针对 CD4+/CD8+,最佳效果概率排序依次为督灸、隔物灸、热敏灸、温针灸、雷火灸、艾灸、麦粒灸、益气养阴汤、益气健脾方、常规治疗.结论:综合各指标排序及临床应用情况,热敏灸在改善肺癌患者免疫功能方面的临床效果较好,益气类中药汤剂次之,临床中可优先选择热敏灸辅以益气类中药汤剂进行治疗.受纳入文献未考虑证型辨治和样本量的限制,后续有待高质量随机对照试验进一步验证.
Objective:By conducting a network meta-analysis,to rank the efficacy of various moxibustion therapies on immune function in lung cancer patients and identify the optimal intervention from the perspective of evidence-based medicine.Methods:Randomized controlled trials(RCTs)evaluating the impact of moxibustion on immune function in lung cancer were retrieved from CNKI,Wanfang,VIP,CBM,PubMed,Web of Science,Embase,CINAHL,and Cochrane Library,from their inception to March 20,2025.Risk of bias was assessed using RoB 2.0 criteria.A Bayesian network Meta-analysis was performed using RevMan 5.4 and Stata 18.Results:Seventeen studies involving 1 472 patients and 7 types of moxibustion were included,with treatment durations ranging from 14 to 105 days.For CD3+,the probability ranking of interventions was:heat-sensitive moxibustion,wheat-grain moxibustion,the Yiqi Yangyin decoction(益气养阴汤),thunder-fire moxibustion,mugwort moxibustion,warm-needle moxibustion,governor vessel moxibustion,the Yiqi Jianpi Fang(益气健脾方),and routine treatment.For CD4+,the ranking was:the Yiqi Yangyin decoction,heat-sensitive moxibustion,governor vessel moxibustion,warm-needle moxibustion,thunder-fire moxibustion,mugwort moxibustion,wheat-grain moxibustion,the Yiqi Jianpi Fang,and routine treatment.For CD8+,the ranking was:heat-sensitive moxibustion,governor vessel moxibustion,the Yiqi Jianpi Fang,warm-needle moxibustion,wheat-grain moxibustion,thunder-fire moxibustion,mugwort moxibustion,and routine treatment.For CD4+/CD8+,the ranking was:governor vessel moxibustion,indirect moxibustion,heat-sensitive moxibustion,warm-needle moxibustion,thunder-fire moxibustion,mugwort moxibustion,wheat-grain moxibustion,the Yiqi Yangyin decoction,the Yiqi Jianpi Fang,and routine treatment.Conclusion:Across immune markers and clinical applicability,heat-sensitive moxibustion exerted the most favorable effect in enhancing immune function in lung cancer patients,followed by Qi(气)-tonifying herbal decoctions.Clinically,heat-sensitive moxibustion supplemented with Qi-tonifying formulas may be given priority.However,given the limited sample sizes and lack of syndrome differentiation in the included trials,further high-quality RCTs are warranted to confirm the findings.
