目的 探讨新加温胆汤通过腺苷酸活化蛋白激酶(AMPK)/沉默信息调节因子1(SIRT1)/过氧化物酶体增殖物激活受体γ共激活因子1α(PGC-1α)信号通路调控能量代谢干预高脂血症伴心肌缺血的作用机制.方法 将30只SPF级SD大鼠随机分为空白组、模型组、温胆汤组、新加温胆汤组、曲美他嗪组,每组6只.除空白组外,其余组予高脂喂养6周建立大鼠高脂血症模型,随后灌胃相应药物14天.末次灌胃1 h后,除空白组外,腹腔注射垂体后叶素构建大鼠心肌缺血模型.检测大鼠心电图变化;HE染色检测大鼠心肌病理形态改变;ELISA检测大鼠血清心肌损伤标志物肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTnI)、肌红蛋白(MYO)浓度;比色法检测血清总胆固醇(TC)、甘油三酯(TG)、超氧化物歧化酶(SOD)、丙二醛(MDA)水平及心肌三磷酸腺苷(ATP)、Na+K+-ATP、Ca2+-ATP酶活性;Real-time PCR 检测心肌 AMPK、SIRT1 及 PGC-1α mRNA 表达;Western Blot 检测心肌 AMPK、p-AMPK、SIRT1、PGC-1α蛋白表达.结果 模型组大鼠心电图J点偏倚,T波高耸,心率加快,心肌纤维断裂,细胞排列紊乱;干预组不同程度改善.与空白组比较,模型组心电图J点抬高值增加,血清中CK-MB、cTnI、MYO、TC、TG、MDA 水平升高,血清 SOD 水平及心肌 ATP、Na+K+-ATP、Ca2+-ATP 酶活性、AMPK、SIRT1及PGC-1αmRNA及蛋白表达降低(P<0.05,P<0.01).与模型组比较,干预组心电图J点抬高值、血清CK-MB、cTnI、MYO水平降低,血清SOD水平及心肌ATP、Na+K+-ATP、Ca2+-ATP酶活性、心肌SIRT1 mRNA、p-AMPK、SIRT1、PGC-1α蛋白表达升高(P<0.05,P<0.01).与温胆汤组比较,新加温胆汤组心肌ATP水平、心肌PGC-1α蛋白表达升高(P<0.05,P<0.01).结论 新加温胆汤能有效降低大鼠心肌损伤,降低血脂水平,调节心肌能量代谢,减轻氧化应激,其潜在机制可能与调节AMPK/SIRT1/PGC-1α信号通路有关.

Objective To investigate the mechanism of Modified Wendan Decoction in intervening hyperlipidemia complicated with myocardial ischemia by modulating energy metabolism through the AMP-activated protein kinase(AMPK)/sirtuin 1(SIRT1)/peroxisome proliferatoractivated receptor gamma coactivator 1-alpha(PGC-1α)signaling pathway.Methods Thirty SPF SD rats were randomly divided into the blank group,the model group,the Wendan Decoction group,the Modified Wendan Decoction group,and the trimetazidine group,6 in each group.Except for the blank group,the other groups received a high-lipid diet for 6 weeks to establish the hyperlipidemia model of rats,and then the corresponding drugs were administered for 14 days.One hour after the final administration,the myocardial ischemia model was induced via intraperitoneal injection of pituitrin,except the blank group.Electrocardiogram was used to detect changes in cardiac electrical activity;HE staining was utilized to assess pathological modifications in rat myocardium;ELISA was conducted to evaluate changes in serum levels of myocardial injury markers creatine kinase-myocardial band(CK-MB),cardiac troponin I(cTnI),and myoglobin(MYO);the levels of total cholesterol(TC),triglycerides(TG),superoxide dismutase(SOD),and malondialdehyde(MDA)in serum,as well as adenosine triphosphate(ATP),Na+K+-ATP,and Ca2+-ATP in myocardial tissue were measured using colorimetry.The mRNA expressions of AMPK,SIRT1,and PGC-1α in myocardial tissue were quantified through Real-time PCR.The AMPK,p-AMPK,SIRT1,and PGC-1α protein expressions in myocardial tissue were analyzed via Western Blot.Results The model group rats showed significant J-point deviation,T-wave elevation,increased heart rate,disrupted myocardial fibers,and disordered cell arrangement;these pathological manifestations were ameliorated to varying degrees in all intervention groups.Compared to the blank group,the model group showed increased J-point deviation and elevated serum levels of CK-MB,cTnI,MYO,TC,TG,and MDA,while serum SOD levels,myocardial ATP content,Na+K+-ATPase and Ca2+-ATPase activities,and AMPK,SIRT1,and PGC-1α mRNA and protein expression were decreased(P<0.05,P<0.01).Compared with the model group,the intervention groups showed decreased J-point deviation and lower serum CK-MB,cTnI,and MYO levels,while serum SOD levels,myocardial ATP content,Na+K+-ATPase and Ca2+-ATPase activities,myocardial SIRT1 mRNA expression,and p-AMPK,SIRT1,and PGC-1α protein expression were increased(P<0.05,P<0.01).Compared with the Wendan Decoction group,the Modified Wendan Decoction group showed higher myocardial ATP content and PGC-1α protein expression(P<0.05,P<0.01).Conclusions Modified Wendan Decoction effectively mitigated myocardial injury,lowered blood lipid levels,regulated myocardial energy metabolism,and alleviated oxidative stress in rats,with its underlying mechanism potentially linked to the modulation of the AMPK/SIRT1/PGC-1α signaling pathway.
