目的:观察急性缺血性脑卒中后认知障碍对言语工作记忆及视觉空间工作记忆容量的影响.方法:选取2021年10月—2023 年 12 月东莞市松山湖中心医院收治的 100 例急性缺血性脑卒中后认知障碍患者为试验组,同时期在社会招募 100 名认知正常者为对照组,两组均进行言语工作记忆容量测试及视觉空间工作记忆容量测试.采用数字广度测试评估言语工作记忆容量,柯西块测试评估视觉空间工作记忆容量.比较两组差异.结果:两组言语工作记忆容量顺序背数得分均高于倒序背数得分,差异有统计学意义(P<0.05);对照组顺序背数、倒序背数得分均高于试验组,差异有统计学意义(P<0.05);两组视觉空间工作记忆容量比较,对照组空间前行、空间倒行得分均高于试验组,差异有统计学意义(P<0.05);两组空间前行得分均高于空间倒行得分,差异有统计学意义(P<0.05).结论:急性缺血性脑卒中后认知障碍影响言语及视觉空间工作记忆容量,导致言语及视觉空间工作记忆容量下降.

Objective:To observe the effect of cognitive impairment on the capacity of verbal working memory(VWM)and visual-spatial working memory(VSWM)after acute ischemic stroke.Method:100 patients with post-stroke cognitive impairment after acute ischemic stroke who were admitted to Dongguan Songshan Lake Central Hospital from October 2021 to December 2023 were selected in the experimental group,and 100 cognitively normal individuals from the society were in the control group.Both groups underwent verbal working memory capacity testing and visual-spatial working memory capacity testing.Digit span test was conducted to assess verbal working memory capacity,and Corsi block test was conducted to assess visual-spatial working memory capacity.The differences between two groups were compared.Result:The scores of sequential memorization of speech working memory capacity in both groups were higher than those of reverse memorization,and the difference was statistically significant(P<0.05).The scores of sequential recitation and reverse recitation in the control group were both higher than those in the experimental group,and the difference was statistically significant(P<0.05).The comparison of visuospatial working memory capacity between the two groups showed that the scores of spatial forward movement and spatial backward movement in the control group were both higher than those in the experimental group,and the difference was statistically significant(P<0.05).The scores of spatial forward movement in both groups were higher than those of spatial backward movement,and the difference was statistically significant(P<0.05).Conclusion:Cognitive impairment after acute ischemic stroke affects speech and visual spatial working memory capacity,leading to a decrease in speech and visual spatial working memory capacity.
