尿液mAlB、β2-MG、TRF和ACR在早期糖尿病肾病中的诊断价值OA
Diagnostic Value of Urinary mAlB,β2-MG,TRF and ACR in Early Diabetic Nephropathy
目的:分析尿微量白蛋白(mAlB)、β2-微球蛋白(β2-MG)、转铁蛋白(TRF)和尿白蛋白/肌酐比值(ACR)在早期糖尿病肾病(DN)中的诊断价值.方法:选取 2020 年 1 月—2025 年 5 月兰州大学第一医院收治的DN患者(观察组,100 例),同期收治的 2 型糖尿病患者(对照组,100 例)为研究对象,采集两组晨尿进行检测,比较尿液mAlB、β2-MG、TRF和ACR水平,通过多因素logistic回归分析探究DN发生的危险因素,通过受试者工作曲线(ROC)和列线图模型评价上述指标单独或联合检测的诊断价值.结果:DN患者尿液中mAlB、β2-MG、TRF和ACR的水平均明显提高,且mAlB、TRF和ACR糖尿病患者发生糖尿病肾病相关.ROC曲线表明mAlB、TRF和ACR联合检测的诊断价值高于单项检测,列线图模型表明三者的预测结果可靠.结论:尿液中mAlB、TRF和ACR水平的升高与DN发生相关,三者联合检测的诊断价值高于单项诊断.
Objective:To analyze the diagnostic value of microalbuminuria(mAlB),β2-microglobulin(β2-MG),transferrin(TRF),and albumin-to-creatinine ratio(ACR)in early-stage diabetic nephropathy(DN).Method:A total of 100 patients with type 2 diabetes mellitus(T2DM)and 100 patients with DN were respectively enrolled as the observation group and control group.Morning urine samples were collected for detection of mAlB,β2-MG,TRF and ACR levels.Multivariate logistic regression analysis was performed to identify risk factors for DN development.The diagnostic values of individual and combined biomarkers were evaluated using receiver operating characteristic(ROC)curve and nomogram modeling.Result:Levels of mAlB,β2-MG,TRF and ACR were significantly elevated in the urine of patients with DN,with mAlB,TRF and ACR showing significant associations with DN development in patients with T2DM.ROC curve demonstrated that the combined detection of mAlB,TRF and ACR exhibited superior diagnostic value compared to individual markers,while the nomogram model confirmed the reliability of their predictive performance.Conclusion:The elevation of urinary mAlB,TRF and ACR are associated with the development of DN,and their combined detection demonstrates superior diagnostic value compared to individual measurements.
