[Objective]To summarize Professor LI Zhaofu's experience in treating rheumatoid arthritis with vine-derived herbal medicines based on"collateral disease theory".[Methods]Through clinical observation under mentorship and analysis of representative medical case studies,this paper summarizes Professor LI's clinical experience in treating rheumatoid arthritis from overview on the collateral disease theory,the core pathogenesis of rheumatoid arthritis,and the diagnostic and therapeutic approach guided by this theory,and a medical record is attached for verification.[Results]Professor LI points out that the fundamental pathogenesis of rheumatoid arthritis is"obstruction of collateral leading to arthralgia",proposing the therapeutic principle that"collateral requires dredging".In clinical application of vine-derived herbal medicines,Professor LI follows the principles of selecting herbs based on syndrome differentiation related to cold,heat,deficiency and excess;grounded in the collateral disease theory,using herbs throughout the entire treatment process;being cautious with the use of toxic vine-derived herbal medicines,emphasizing warming Yang to dredge the collateral.In the initial consultation,Professor LI diagnosed the patient as wind-cold-damp arthralgia syndrome,based on the onset of symptoms after fatigue and sweating,treated with Wenyang Tongluo Formula according to principles of warming Yang and dispelling cold,expelling wind and eliminating dampness,and promoting circulation to relieve pain.During the course of treatment,vine-derived herbal medicines were selected to facilitate pathways,with cautious use of Polygoni Multiflori Caulis,adjusting the dosage according to the patient's condition,and achieving good therapeutic effects.[Conclusion]Professor LI treats rheumatoid arthritis based on the collateral disease theory,adhering to the treatment principle of"collateral requires dredging",taking Wenyang Tongluo Formula as the basic prescription,adjusting herbs according to the patient's different conditions.Professor LI's experience enriches the system of syndrome differentiation and treatment for rheumatoid arthritis,and is worthy of further promotion and research.
