乙胺丁醇视神经毒性是抗结核治疗中不可逆视力损伤的重要原因,临床常表现为亚急性、双眼对称、无痛的视功能下降,其机制源于药物及代谢产物2,2'-(乙二胺基)-二丁酸螯合铜锌离子,引发线粒体-溶酶体双重功能障碍、三磷酸腺苷耗竭、轴突运输阻滞,最终导致视网膜神经节细胞凋亡;亚临床阶段视网膜神经纤维层和视网膜神经节细胞层-内丛状层的厚度变化及视觉诱发电位P100潜伏期延长已同步出现,黄斑血管密度下降更早提示血流受损,这不仅为乙胺丁醇视神经毒性的早期诊断提供了重要依据,也为探索其发病机制提供了重要线索,而每日剂量≥15 mg/kg、疗程>6个月、慢性肾病、高龄、吸烟等显著增加风险,临床医师熟练掌握相关的发病机制、眼部改变和危险因素,将有助于早期诊断和治疗,同时也能最大程度地避免患者发生不可逆的视力损伤.在确保抗结核疗效的同时减少不可逆视力损伤,本文系统梳理乙胺丁醇视神经毒性的机制、结构-功能损害特征与高危因素,旨在为临床早期识别与精准防控提供循证依据与研究方向.

Ethambutol-induced optic neurotoxicity is a major cause of irreversible visual impairment during anti-tuberculosis treatment,clinically characterized by subacute,bilateral symmetric and painless visual dysfunction.Its pathogenic mechanism stems from the chelation of copper and zinc ions by ethambutol and its metabolite 2,2'-(ethylenediamino)-dibutyric acid,which triggers dual mitochondrial-lysosomal dysfunction,adenosine triphosphate depletion and axonal transport blockage,ultimately leading to the apoptosis of retinal ganglion cells.At the subclinical stage,thickness changes in the retinal nerve fiber layer and the retinal ganglion cell inner plexiform layer,as well as prolonged P100 latency of visual evoked potentials,occur synchronously;the earlier decrease in macular vascular density suggests impaired ocular blood flow.These changes not only provide critical evidence for the early diagnosis of ethambutol-induced optic neurotoxicity but also offer important clues for exploring its pathogenesis.Moreover,risk factors for this condition include a daily dosage of≥15 mg/kg,a treatment course exceeding 6 months,chronic kidney disease,advanced age and smoking,all of which significantly increase the incidence risk.Clinicians'proficient grasp of the relevant pathogenesis,ocular changes and risk factors will facilitate early diagnosis and treatment,and to the greatest extent prevent irreversible visual impairment in patients.To reduce irreversible visual impairment while ensuring the efficacy of anti-tuberculosis therapy,this article systematically summarizes the pathogenesis,structural and functional damage characteristics,and high-risk factors of ethambutol-induced optic neurotoxicity,aiming to provide evidence-based basis and research directions for its early clinical identification and precise prevention and control.
