双侧与单侧人工耳蜗佩戴下噪音聆听任务对唾液皮质醇浓度的影响OA
Effects of bilateral versus unilateral cochlear implant use on salivary cortisol concentration during listening tasks under noise in patients with bilateral sensorineural hearing loss
目的 分析双侧人工耳蜗植入患者在3种不同条件下,唾液中皮质醇水平的变化,为听觉疲劳和认知负荷的评估提供新的方法学依据.方法 纳入北京市顺义区妇幼保健院2025年5月收治的双侧人工耳蜗植入患者15例,其中男性9例,女性6例,年龄3.8~47.3岁,平均年龄(10.83±10.49)岁.所有患者唾液样本均在14:00后采集,以排除皮质醇昼夜节律变化的影响.样本采集分别在无任务的基线状态下完成、噪音环境下双侧人工耳蜗佩戴时完成、噪音环境下单侧人工耳蜗佩戴时完成.听力任务在-15 dB HL信噪比的噪音环境中进行,持续时间为15 min.结果 通过配对t检验发现,双侧听力任务后与基线状态相比,皮质醇水平无显著差异(P=0.488).然而,单侧听力任务后皮质醇水平显著高于双侧任务后(P=0.017)和基线状态(P=0.048).结论 在噪音环境下单侧人工耳蜗佩戴可能会引起更强烈的生理应激反应,提示在临床康复计划中应重视听觉负荷对患者生理和心理状态的影响.
Objective To analyze cortisol levels in saliva in bilateral cochlear implant recipients under three distinct conditions,providing novel methodological evidence for the assessment of auditory fatigue and cognitive load.Methods This study included 15 bilateral cochlear implant recipients(6 females and 9 males,aged 3.8~47.3 years[mean=10.83±10.49 years])who underwent programming in May 2025.Saliva samples were collected after 2:00 PM to eliminate the influence of circadian rhythm cortisol secretion.Samples were collected under three conditions:baseline state(no task),bilateral cochlear implants use in noise,and unilateral cochlear implant use in noise.The auditory task was performed in a noise environment with a signal-to-noise ratio of-15 dB HL for 15 minutes.Results Paired t-tests revealed no significant difference in cortisol levels between the bilateral auditory task and the baseline state(P=0.488).However,cortisol levels were significantly higher after the unilateral auditory task compared to both the bilateral task(P=0.017)and the baseline state(P=0.048).Conclusions Unilateral cochlear implant use in noisy environments may induce a pronounced physiological stress responses,suggesting that the impact of auditory load on patients'physiological and psychological state should be a key consideration in clinical rehabilitation programs.
