目的 研究中耳胆脂瘤上皮组织BAG-1表达与病变侵袭程度及鼓室成形术后残留和复发的相关性.方法 纳入2022年10月1日至2024年5月1日在潍坊市人民医院接受治疗的中耳胆脂瘤患者125例为观察组,选取同期在潍坊市人民医院接受治疗的耳外伤、耳畸形、耳部感染等非胆脂瘤患者125例为对照组.观察组根据听小骨破坏程度分为0级组、1级组、2级组、3级组,根据STAM分期系统分为Ⅰ期组、Ⅱ期组、Ⅲ期组,根据术后情况分为非术后残留和复发组、术后残留和复发组.检测观察组和对照组上皮组织BAG-1表达情况.比较观察组不同听小骨破坏程度、不同浸润情况、不同术后残留和复发的BAG-1表达差异.结果 观察组BAG-1表达高于对照组,差异有统计学意义(P<0.05);BAG-1表达随听小骨损害程度加重、浸润程度加重而升高,差异均有统计学意义(P<0.05);非术后残留和复发组BAG-1光密度值低于术后残留和复发组,差异有统计学意义(P<0.05);BAG-1表达情况与残留和复发、浸润情况呈现正相关,差异有统计学意义(P<0.05).结论 中耳胆脂瘤上皮组织BAG-1表达与胆脂瘤侵袭程度及鼓室成形术术后残留、复发呈正相关,可作为临床诊断的重要依据.

Objective To investigate the association between BAG-1 expression in middle ear cholesteatoma epithelial tissue and the extent of lesion invasion,as well as postoperative residual and recurrent disease following tympanoplasty.Methods A total of 125 patients with middle ear cholesteatoma treated at our hospital between October 1,2022 and May 1,2024 were included.Based on ossicular chain destruction,16 patients were classified as grade 0,29 as grade 1,31 as grade 2,and 49 as grade 3.According to the STAM staging system,42 patients were stageⅠ,58 stageⅡ,and 25 stage Ⅲ.Thirty patients experienced postoperative residual or recurrent disease.An additional 125 patients treated during the same period for non-cholesteatoma ear conditions(e.g.trauma,malformation,infection)served as the control group.Compare the differences in BAG-1 expression among the observation group with different degrees of ossicular destruction,different infiltration conditions,and different postoperative residuals and recurrences.Results BAG-1 expression was significantly elevated in the cholesteatoma group compared with the control group(P<0.05).BAG-1 levels progressively increased with greater ossicular chain destruction and deeper lesion infiltration(P<0.05).Patients without residual or recurrent lesions exhibited markedly lower BAG-1 optical density value than those with residual or recurrent disease(P<0.05).BAG-1 expression showed a significant positive correlation with both the extent of cholesteatoma invasion and postoperative residual/recurrent lesions(P<0.05).Conclusions BAG-1 expression in middle ear cholesteatoma epithelial tissue is positively associated with lesion invasiveness and postoperative residual or recurrent disease after tympanoplasty.These findings suggest that it may serve as valuable biomarker for assessing disease severity and predicting postoperative outcomes in cholesteatoma.
