基于英国生物银行数据的人工耳蜗植入横断面研究OA

A cross-sectional study of cochlear implantation based on data from the UK Biobank

目的 探讨人工耳蜗植入对听力损失患者耳鸣、睡眠及心理健康等相关非听觉结局的影响.方法 基于英国生物银行数据,筛选出人工耳蜗植入者(CI组,n=197)及符合植入候选标准但未植入的听力损失个体(候选组,n=668).采用横断面分析,比较两组在基线特征、耳鸣、睡眠困难及心理健康等方面的差异.采用多重插补法处理缺失数据,并利用多变量逻辑回归及倾向评分匹配(1∶1)控制性别、年龄混杂因素,分析人工耳蜗植入与各健康结局的相关性.结果 在调整性别和年龄后,与候选组相比,CI组神经质评分显著更低(调整后OR=0.56,95%CI:0.38~0.81,P=0.002),失眠风险亦较低(调整后OR=0.65,95%CI:0.44~0.94,P=0.024);然而,人工耳蜗植入与耳鸣、睡眠时间、抑郁发作、焦虑症等无显著关联.倾向评分匹配分析进一步支持人工耳蜗植入与较低神经质评分的关联(OR=0.63,95%CI:0.40~0.99,P=0.044),但与失眠的关联不再显著.结论 人工耳蜗植入可能与情绪稳定性改善及睡眠质量提升相关,但与耳鸣及临床诊断的心境障碍无稳定关联.结果 提示人工耳蜗的获益可能延伸至听觉功能以外的心理健康维度,但需前瞻性研究验证因果效应.

Objective To evaluate the impact of cochlear implantation(CI)on non-auditory outcomes,including tinnitus,sleep,and mental health,among individuals with hearing loss.Methods Using data from the UK Biobank(UKB),we identified cochlear implants users(CI group,n=197)and hearing-impaired individuals who met the candidacy criteria for implantation but had not received an implant(candidate group,n=668).In this cross-sectional analysis,we compared baseline characteristics,tinnitus,sleep difficulties and mental health indicators between groups.Missing data were addressed through multiple imputation.Multivariable logistic regression and propensity score matching(1:1)were applied to adjust for confounding by sex and age,and to examine association between CI implantation use and health outcomes.Results After adjusting for sex and age,CI users demonstrated significantly lower neuroticism scores(adjusted OR=0.56,95%CI:0.38~0.81,P=0.002)and a lower risk of insomnia(adjusted OR=0.65,95%CI:0.44~0.94,P=0.024)compared candidates.No significant associations with CI use were observed for tinnitus,sleep duration,depressive episodes,anxiety disorders,etc.Propensity score matching analysis confirmed the association between CI use and lower neuroticism(OR=0.63,95%CI:0.40~0.99,P=0.044),while the association with insomnia was no longer significant.Conclusions This study suggests that cochlear implantation may be associated with improved emotional stability and sleep quality,although no consistent associations were found for tinnitus or clinically diagnosed mood disorders.These findings indicate that the benefits of CI may extend beyond auditory restoration to certain dimensions of mental well-being.Prospective studies are needed to verify causal effects.
