卒中是导致成人长期残疾的重要病因,其康复是贯穿急性期、亚急性期、慢性期的动态过程,各阶段具有独特的神经生理学特征与康复需求.康复脑机接口(rehabilitation brain-computer interface,rBCI)技术通过解码卒中患者的大脑运动意图,建立大脑与外部设备间的直接通信通路,为卒中后功能障碍康复提供了创新性策略.然而,rBCI技术在卒中康复中的临床应用,要求其干预范式、信号采集、特征提取与实时反馈方式可与病程的动态演变实现精准适配.本文系统综述了rBCI技术针对卒中不同病程(急性期、亚急性期、慢性期)特点的适配与优化策略:首先阐述卒中后神经可塑性时间窗及其在各阶段的病理生理学特征;其次重点分析rBCI技术在急性期用于监测、评估与早期被动干预,在亚急性期用于主动感觉运动环路重塑,在慢性期用于功能优化与应用场景泛化等方面的应用策略、技术特点和演进趋势.本综述旨在为临床医师与科研人员提供清晰的理论框架,助力其理解并推动rBCI技术在卒中全周期康复中的动态应用.

Stroke is a leading cause of long-term disability in adults.Its rehabilitation is a dynamic process spanning the acute,subacute,and chronic phases,each with distinct neurophysiological characteristics and rehabilitation needs.Rehabilitation brain-computer interface(rBCI)technologies decode motor intentions from the brain of stroke patients and establish a direct communication pathway between the brain and external devices,providing innovative strategies for the rehabilitation of post-stroke dysfunction.However,the clinical application of rBCI in stroke rehabilitation requires that its intervention paradigms,signal acquisition,feature extraction,and real-time feedback be precisely matched to the dynamic course of stroke.This paper systematically reviews the adaptation and optimization strategies of rBCI tailored to the characteristics of different stroke phases(acute,subacute,and chronic phases).First,it describes the time windows of post-stroke neuroplasticity and the pathophysiological features of each phase.Second,it focuses on the application strategies,technical characteristics,and developmental trends of rBCI,specifically addressing monitoring,assessment,and early passive intervention in the acute phase;active sensorimotor circuit remodeling in the subacute phase;and functional optimization and generalization of application scenarios in the chronic phase.This review aims to provide a clear theoretical framework for clinicians and researchers,facilitating the understanding and advancement of the dynamic application of rBCI in the full-cycle rehabilitation of stroke.
