伏立康唑滴眼液与那他霉素滴眼液分别联合角膜溃疡清创术治疗真菌性角膜炎的疗效及对菌丝浸润深度影响的对比分析研究OA
Comparative Analysis of the Efficacy of Voriconazole Eye Drops and Natamycin Eye Drops Combined with Corneal Ulcer Debridement in the Treatment of Fungal Keratitis and its Effect on Hyphal Infiltration Depth
目的:对比分析伏立康唑滴眼液与那他霉素滴眼液分别联合角膜溃疡清创术治疗真菌性角膜炎(FK)患者的临床疗效及对菌丝浸润深度的影响.方法:回顾性分析2019年6月～2024年6月本院收治的80例接受角膜溃疡清创术的FK患者病历资料,根据不同治疗方法分为对照组(n=38)和观察组(n=42).两组患者术后同时给予左氧氟沙星滴眼液进行基础治疗,对照组给予那他霉素滴眼液治疗,观察组给予伏立康唑滴眼液治疗,两组均连续治疗4周.比较两组临床疗效、菌丝浸润深度、眼部总评分、视力情况及不良反应发生情况.结果:治疗4周后,观察组临床治疗总有效率(90.48%)高于对照组(73.68%,P＜0.05);菌丝浸润深度及眼部总评分均低于对照组(P＜0.05);视力情况优于对照组(P＜0.05).两组不良反应总发生率比较无统计学差异(χ2=0.339,P=0.560).结论:在角膜溃疡清创术治疗基础上,给予伏立康唑滴眼液可有效提高FK患者临床疗效,改善菌丝浸润深度、眼部总评分及视力.
Objective:To compare the clinical efficacy of voriconazole eye drops versus natamycin eye drops combined with corneal ulcer debridement in the treatment of fungal keratitis(FK)and its effect on hyphal infiltration depth.Methods:Medical records of 80 FK patients who underwent corneal ulcer debridement in our hospital from June 2019 to June 2024 were retrospectively analyzed.They were divided into the control group(n=38)and the observation group(n=42)according to different treatments.Both groups received levofloxacin eye drops as basic treatment after surgery.The control group was given natamycin eye drops,and the observation group was given voriconazole eye drops.Both groups were treated continuously for 4 weeks.Clinical efficacy,hyphal infiltration depth,total ocular score,visual acuity and adverse reactions were compared between the two groups.Results:After 4 weeks of treatment,the total clinical effective rate in the observation group(90.48%)was higher than that in the control group(73.68%,P<0.05).Hyphal infiltration depth and total ocular score in the observation group were lower than those in the control group(P<0.05).Visual acuity in the observation group was better than that in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(χ2=0.339,P=0.560).Conclusion:On the basis of corneal ulcer debridement,voriconazole eye drops can effectively improve the clinical efficacy of FK patients,reduce hyphal infiltration depth and total ocular score,and improve visual acuity.
