红核妇洁洗液联合氟康唑胶囊及乳酸菌阴道胶囊治疗霉菌性阴道炎的疗效与安全性观察OA
Efficacy and Safety of Honghe Fujie Lotion Combined with Fluconazole Capsules and Lacidophilin Vaginal Capsules in the Treatment of Vulvovaginal Candidiasis
目的:探讨红核妇洁洗液联合氟康唑胶囊及乳酸菌阴道胶囊治疗霉菌性阴道炎的疗效及安全性.方法:选取 2022 年 6 月～2024 年 5 月本院收治的116 例霉菌性阴道炎患者,采用随机数字表法分为对照组(因依从性差剔除1例,因主动退出研究剔除 2 例,n=55)和观察组(因依从性差剔除 3 例,因检查治疗不完整剔除1例,n=54).对照组给予氟康唑胶囊、乳酸菌阴道胶囊治疗,观察组在对照组基础上给予红核妇洁洗液治疗,两组均连续用药 7 天.比较两组患者阴道微生态指标(唾液酸酶、白细胞酯酶、过氧化氢酶的阳性率)、阴道 pH 值、炎症指标[白介素-2(IL-2)、白介素-13(IL-13)、肿瘤坏死因子-α(TNF-α)、单核细胞趋化蛋白1(MCP-1)]、霉菌性阴道炎评分、复发情况及不良反应发生情况.结果:治疗后,两组唾液酸酶、白细胞酯酶和过氧化氢酶的阳性率均降低,但两组比较均无统计学差异(P＞0.05).两组阴道 pH值及 IL-13、TNF-α、MCP-1水平均降低,且观察组低于对照组(P＜0.05);IL-2 水平均升高,且观察组高于对照组(P＜0.05);瘙痒、疼痛、充血/水肿、抓痕/糜烂/皲裂、分泌物量以及霉菌性阴道炎总评分均降低,且观察组低于对照组(P＜0.05).观察组治疗后 6 个月的复发率(5.56%)低于对照组(18.18%,P＜0.05).两组治疗后 3 个月的复发率及不良反应总发生率比较均无统计学差异(P＞0.05).结论:红核妇洁洗液联合氟康唑胶囊及乳酸菌阴道胶囊治疗霉菌性阴道炎可有效改善阴道微生态,减轻症状,调节炎症指标水平,降低复发率.
Objective:To explore the efficacy and safety of Honghe Fujie Lotion combined with fluconazole capsules and lacidophilin vaginal capsules in the treatment of vulvovaginal candidiasis.Methods:A total of 116 patients with vulvovaginal candidiasis admitted to our hospital from June 2022 to May 2024 were selected and divided into the control group and the observation group by random number table method.One case was excluded from the control group due to poor compliance,and 2 cases due to active withdrawal(finally n=55);3 cases were excluded from the observation group due to poor compliance,and 1 case due to incomplete examination and treatment(finally n=54).The control group was treated with fluconazole capsules and lacidophilin vaginal capsules,while the observation group was additionally given Honghe Fujie Lotion on the basis of the control group's treatment.Both groups received continuous medication for 7 days.Vaginal microecological indicators(positive rates of sialidase,leukocyte esterase and catalase),vaginal pH,inflammatory indicators[interleukin-2(IL-2),interleukin-13(IL-13),tumor necrosis factor-α(TNF-α),monocyte chemoattractant protein-1(MCP-1)],vulvovaginal candidiasis symptom scores,recurrence and adverse reactions were compared between the two groups.Results:After treatment,the positive rates of sialidase,leukocyte esterase and catalase decreased in both groups,with no statistically significant difference between the two groups(P>0.05).The vaginal pH,levels of IL-13,TNF-α and MCP-1 decreased in both groups,and the observation group had lower values than the control group(P<0.05);the IL-2 level increased in both groups,and the observation group had a higher level than the control group(P<0.05).Scores of itching,pain,congestion/edema,scratch/erosion/chapping,discharge volume,and total vulvovaginal candidiasis score decreased in both groups,and the observation group had lower scores than the control group(P<0.05).The 6-month recurrence rate after treatment in the observation group(5.56%)was lower than that of the control group(18.18%,P<0.05).There were no statistically significant difference in the 3-month recurrence rate after treatment and total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Honghe Fujie Lotion combined with fluconazole capsules and lacidophilin vaginal capsules can effectively improve vaginal microecology,alleviate symptoms,regulate inflammatory indicator levels,and reduce the recurrence rate in the treatment of vulvovaginal candidiasis.
