论伤寒病机理论的起源、形成与发展演变OA
Origin,Formation,Development and Evolution of the Pathogenesis Theory of Cold Damage
历代医家均重视伤寒的研究,但就伤寒病机理论的认识分歧至今.本文通过梳理认为,伤寒病机理论最早发端于秦汉,初步构建于隋唐,形成于宋金元,发展演变于明清.从最早的风寒入腠理,与正气交争,营卫运行不畅,三日以上邪气在上焦,五日以上邪气在脏腑之说;发展形成了寒郁化热说、寒伤营卫说、寒极生热说、阴证伤寒说;后逐渐演变至风伤卫、寒伤营、风寒两伤营卫说,六气说、标本中气说等.自明清以来,医家研究伤寒病机更关注基于《伤寒论》文本的理论探讨,未来研究亟待突破文本研究的局限,构建系统的伤寒病机理论体系.
Medical practitioners throughout history have attached great importance to the study of cold damage,yet there remains disagreement on the understanding of its pathogenesis theory.This article concludes through analysis and review that the pathogenesis theory of cold damage originated in the Qin and Han dynasties,was initially constructed in the Sui and Tang dynasties,took shape in the Song,Jin,and Yuan dynasties,and evolved in the Ming and Qing dynasties.From the earliest theory of wind-cold entering the skin and muscle,competing with vital Qi,causing the circulation of the Ying(nutrient)and Wei(defense)to be obstructed,with pathogenic Qi accumulating in the upper energizer for more than three days and in the Zang-Fu organs for more than five days;it developed into theories such as cold stagnation turning into heat,cold injuring the Ying and Wei,extreme cold generating heat,and Yin syndrome cold damage;later,it gradually evolved into theories of wind injuring the Wei,cold injuring the Ying,wind and cold injuring both the Ying and Wei,the six Qi theory,and the theory of root,branch,and middle Qi.Since the Ming and Qing dynasties,medical practitioners have paid more attention to theoretical discussions based on the text of Treatise on Febrile Diseases in studying the pathogenesis of cold damage.Future research urgently needs to break through the limitations of textual research and construct a systematic theoretical system of cold damage pathogenesis.
