基于多指标决策分析的针刺治疗慢性阻塞性肺疾病合并膈肌功能障碍选穴方案探析OA
An analysis of acupuncture point selection for treating chronic obstructive pulmonary disease with diaphragmatic dysfunction based on multi-index decision analysis
目的 筛选当前针刺治疗慢性阻塞性肺疾病(COPD)合并膈肌功能障碍的优势方案,为今后的临床研究提供参考依据.方法 检索中国知网(CNKI)、维普数据库、万方数据库、美国国立医学图书馆PubMed 数据库收录的 2015 年 7 月 1 日到 2025 年 7 月 1 日以针刺为主要干预措施,以 COPD 合并膈肌功能障碍的患者为干预对象,且结局指标中包含了膈肌功能指标的临床研究文献.采用标准化均数差(SMD)计算出膈肌功能指标效应值,并运用多指标决策分析法筛选优势方案.结果 共纳入 12 篇文献,筛选出的优势方案为在机械通气和基础治疗基础上加用针刺定喘、肺俞、膻中、鸠尾、中脘、太渊、足三里、丰隆等穴位治疗,留针30 min,每日 1 次,共治疗 10 d.纳入方案显示,干预方式以毫针为主,取穴以任脉、背俞穴居多.结论 针灸治疗 COPD 合并膈肌功能障碍应注重肺脾同调并结合背俞穴使用,此方案可为临床治疗 COPD 合并膈肌功能障碍提供治疗思路.
Objective To obtain the optimal intervention method of acupuncture for chronic obstructive pulmonary disease(COPD)with diaphragmatic dysfunction,and to provide evidence for future clinical research.Methods Clinical studies were retrieved from China National Knowledge Infrastructure(CNKI),VIP Database,Wanfang Database,and PubMed,from July 1,2015,to July 1,2025.The studies focused on acupuncture as the main intervention,patients with COPD and diaphragmatic dysfunction as the intervention object,and the outcome indicators included diaphragmatic function parameters The standardized mean difference(SMD)was used to calculate the effect value of diaphragmatic function indicators,and the multi-criteria decision analysis was used to screen the superior scheme.Results A total of 12 articles were included.The screened superior scheme was acupuncture at Dingchuan(EX-B1),Feishu(BL13),Danzhong(CV17),Jiuwei(CV15),Zhongwan(CV12),Taiyuan(LU9),Zusanli(ST36),and Fenglong(ST40)for 30 minutes of needle retention,once a day for 10 days,in addition to mechanical ventilation and basic treatment.The inclusion scheme indicated that the intervention was mainly based on filiform needle,and the acupoints were mostly selected from Conception vessel and Backshu points.Conclusion Acupuncture and moxibustion treatment of COPD with diaphragmaticdysfunction should emphasize regulating both lung and spleen and combine with the use of Backshu points.This scheme can provide a therapeutic approach for the clinical treatment of COPD with diaphragmatic dysfunction.
