从"自主神经-免疫"调控解析针刺改善炎症性肠病机制的研究思路探讨OA
Approaches to the mechanism research of acupuncture for attenuating inflammatory bowel disease from the perspective of"autonomic nerve-immune"regulation
针刺作为一种体表物理干预手段,可通过调控包括交感神经、迷走神经在内的自主神经传出活动,影响多种神经递质的释放,实现对效应器官动力、代谢和免疫等功能的调节.近年来,神经系统对炎症性肠病(IBD)发生与发展的调控作用日益受到重视.尽管高质量证据揭示了针刺通过"神经-免疫"调控改善系统性炎症的潜力,但针刺改善 IBD 区域性炎性反应的"自主神经-免疫"机制研究仍较匮乏.本文基于最新神经科学与免疫学研究进展,系统梳理肠道炎症状态下神经-免疫调控的前沿进展,并结合现阶段针刺抗炎的神经生物学证据,对针刺改善 IBD 潜在的自主神经-免疫调控途径进行深入探讨,倡议立足系统生物学视角,结合多学科交叉技术,揭示"穴位-自主神经-肠道免疫"调控的"全链条"证据,为后续针刺缓解IBD的机制解析提供参考与借鉴.
As a kind of physical intervention on the body surface,acupuncture may regulate the activities of autonomic nerve including sympathetic nerve and vagus nerve,and influence the release of many kinds of neurotransmitters,so as to regulate the dynamic,metabolic and immune functions of the effector organs.In recent years,the regulatory role of the nervous system in the pathogenesis and progression of inflammatory bowel disease(IBD)has garnered increasing attention.Although high-quality evidence has revealed the potential of acupuncture to ameliorate systemic inflammation through"nerve-immune"modulation,the research of the"autonomic nerve-immune"mechanism of acupuncture remains limited for attenuating the regional inflammatory responses in IBD.Based on the latest advancements in neuroscience and immunology,this study systematically reviews the nerve-immune regulatory mechanisms underlying IBD and discusses in-depth the potential autonomic nerve-immune regulatory mechanisms of acupuncture in combination with current neurobiological evidence of its anti-inflammatory effect.It suggests adopting systems biology and integrating multidisciplinary technologies to elucidate the"full-chain"evidence of the"acupoint-autonomic nervous system-intestinal immune"regulatory mechanism,and provides the reference for subsequent mechanistic analysis of acupuncture in alleviating IBD.
