基于代谢组学探讨壮医热敏探穴针刺法干预类风湿关节炎的作用机制OA
Mechanism of Zhuang medicine heat-sensitive acupuncture in treating rheumatoid arthritis based on metabolomics
目的:基于代谢组学探讨壮医热敏探穴针刺法缓解类风湿关节炎(RA)症状的可能作用机制.方法:将35只 SPF级雄性SD大鼠随机分为正常对照组(10只)和造模组(25 只),造模组大鼠于右后足底注射0.1 mL完全弗氏佐剂(1 mg/mL)诱导RA模型,将造模成功的25只大鼠随机分为模型组(10 只)和观察组(15只),观察组予艾灸右侧"镇寒穴",将出现腧穴热敏现象的大鼠纳入壮医热敏探穴针刺组(9 只).壮医热敏探穴针刺组大鼠予针刺右侧"镇寒穴",留针20 min,每日1次,连续干预21 d.分别于实验的第15、25、32、39天测量各组大鼠右侧踝关节直径.干预后,采用 ELISA 法检测大鼠血清白细胞介素(IL)-10 和 IL-17 含量;HE 染色法观察大鼠踝关节组织形态学;基于超高效液相色谱-四极杆-飞行时间串联质谱技术对大鼠粪便样本进行代谢组学分析,筛选出大鼠粪便差异代谢物以及代谢通路.结果:与正常对照组比较,模型组大鼠右后肢足跖发热、肿胀,实验第15、25、32、39天右侧踝关节直径增加(P<0.01);血清IL-10含量降低、IL-17 含量升高(P<0.01);右侧踝关节滑膜损伤明显,可见大量炎症细胞浸润.与模型组比较,壮医热敏探穴针刺组大鼠右后肢足跖肿胀减轻,实验第 25、39 天右侧踝关节直径减小(P<0.05);血清 IL-10 含量升高、IL-17 含量降低(P<0.05);右侧踝关节滑膜组织损伤改善,炎症细胞数量减少.代谢组学共筛选出脱氧肌苷、硫胺素、皮质醇、核黄素和5-羟基吲哚乙酸等19种潜在的生物标志物,涉及氨基酸代谢、嘌呤代谢、类固醇激素生物合成和维生素消化与吸收等通路.结论:壮医热敏探穴针刺法可有效改善 RA 模型大鼠关节肿胀症状,其作用机制可能与调节色氨酸代谢、维生素消化吸收以及类固醇生物合成途径有关.
Objective To investigate the potential mechanism by which the Zhuang medicine heat-sensitive acupuncture method alleviates symptoms of rheumatoid arthritis(RA),based on metabolomics analysis.Methods A total of 35 SPF-grade male SD rats were randomly divided into a normal control group(10 rats)and a modeling group(25 rats).RA models were induced in the modeling group by injecting 0.1 mL of complete Freund's adjuvant(1 mg/mL)into the right hind paw.Successfully modeled 25 rats were then randomly divided into a model group(10 rats)and an observation group(15 rats).The rats in the observation group received moxibustion at the right-side"Zhenhan point";the rats exhibiting heat-sensitive acupoint phenomena were included in the Zhuang medicine heat-sensitive acupuncture group(9 rats).The rats in this group received acupuncture at the right-side"Zhenhan point"with a retention time of 20 minutes,once daily for 21 consecutive days.Ankle joint diameter was measured on days 15,25,32,and 39.After the intervention,ELISA was used to detect serum levels of interleukin(IL)-10 and IL-17;hematoxylin-eosin(HE)staining was employed to observe the histomorphology of the ankle joint;fecal samples were analyzed using ultra-high performance liquid chromatography coupled with UHPLC-Q-TOF-MS for metabolomics to identify differential fecal metabolites and metabolic pathways.Results Compared with the normal control group,the rats in the model group showed paw hyperthermia and swelling of right paw;right ankle joint diameters were significantly increased on days 15,25,32,and 39(P<0.01);the serum level of IL-10 was decreased and that of IL-17 was increased(P<0.01);significant synovial damage and infiltration of inflammatory cells were observed in the right ankle joint.Compared with the model group,the Zhuang medicine heat-sensitive acupuncture group exhibited reduced paw swelling and decreased ankle joint diameter of right side on days 25 and 39(P<0.05);the serum level of IL-10 was increased and that of IL-17 was decreased(P<0.05);the damage of synovial tissue in the right ankle joint was improved with reduced inflammatory cell infiltration.A total of 19 potential biomarkers were identified via metabolomics,including deoxyinosine,thiamine,cortisol,riboflavin,and 5-hydroxyindoleacetic acid,involving pathways such as amino acid metabolism,purine metabolism,steroid hormone biosynthesis,and vitamin digestion and absorption.Conclusion Zhuang medicine heat-sensitive acupuncture method can effectively alleviate joint swelling in RA model rats.The underlying mechanism may be related to modulation of tryptophan metabolism,vitamin digestion and absorption,and steroid biosynthesis pathways.
