目的:观察热敏灸对甲基苯丙胺成瘾者(戒断期)心理渴求的影响.方法:将 60 例甲基苯丙胺成瘾者(戒断期)随机分为观察组和对照组,各 30 例.对照组接受戒毒所内常规戒毒矫正教育;观察组在对照组基础上于神阙、膻中进行热敏灸治疗,每次 60 min,每天 1 次,每周 4 次,两组均治疗 4 周.于治疗前、治疗4 周后及治疗结束后 2 周(随访)观察两组受试者心理渴求视觉模拟量表(VAS)评分,于治疗前后采用近红外脑功能成像(fNIRS)技术观察两组受试者脑区激活及脑功能连接度变化.结果:治疗 4 周后及随访时,观察组受试者心理渴求 VAS 评分较治疗前降低(P<0.01),且低于对照组(P<0.01).治疗 4 周后,观察组受试者药物刺激下左侧额下回(L-IFG)、左侧额极区(L-FPA)、左侧背外侧前额叶(L-DLPFC)激活较治疗前及对照组减弱(P<0.01,P<0.05).治疗 4 周后,观察组受试者右侧额下回(R-IFG)与左侧前运动皮层和辅助运动皮层(L-PM&SMA)、左侧体感皮层(L-S1)、右侧韦尼克区(R-Wernicke)脑区,右侧布洛卡区(R-Broca)与右侧体感皮层(R-S1)、L-S1 脑区功能连接度增强(P<0.05,P<0.01),对照组受试者 L-FPA 与右侧前运动皮层和辅助运动皮层(R-PM&SMA)脑区功能连接度增强(P<0.05);观察组受试者 R-IFG 与 L-PM&SMA、L-S1、R-Wernicke 脑区功能连接度较对照组增强(P<0.05,P<0.01).结论:热敏灸可减轻甲基苯丙胺成瘾者(戒断期)的心理渴求,其机制可能涉及多个脑区的复杂调节.

Objective To observe the effect of heat-sensitive moxibustion on psychological craving in patients with methamphetamine addiction(withdrawal stage).Methods Sixty patients with methamphetamine addiction(withdrawal stage)were randomly divided into an observation group and a control group,with 30 cases in each group.The control group received routine drug rehabilitation and correctional education in the rehabilitation center;the observation group received heat-sensitive moxibustion at Shenque(CV8)and Danzhong(CV17)on the basis of the control group regimen,60 min per session,once daily,four times a week.Both groups were treated for 4 weeks.The visual analogue scale(VAS)score for psychological craving was assessed before treatment,4 weeks into treatment,and 2 weeks after treatment completion(follow-up)in the two groups.Functional near-infrared spectroscopy(fNIRS)was used to observe the brain region activation and brain functional connectivity changes before and after treatment in the two groups.Results Compared before treatment,the VAS scores for psychological craving were reduced at 4 weeks into treatment and at follow-up in the observation group(P<0.01),which were lower than those in the control group(P<0.01).Compared before treatment and the control group,the brain activation in the left inferior frontal gyrus(L-IFG),left frontopolar area(L-FPA),and left dorsolateral prefrontal cortex(L-DLPFC)was decreased in the observation group(P<0.01,P<0.05).Compared before treatment,the functional connectivity was enhanced between the right inferior frontal gyrus(R-IFG)and left premotor&supplementary motor cortex(L-PM&SMA),left somatosensory cortex(L-S1),right Wernicke(R-Wernicke),as well as the right Brocaʹs area and right somatosensory cortex(R-S1),L-S1 in the observation group after treatment(P<0.05,P<0.01).Compared before treatment,the functional connectivity was enhanced between L-FPA and right PM&SMA in the control group after treatment(P<0.05).Compared with the control group,the observation group showed stronger enhancements in functional connectivity of R-IFG with L-PM&SMA,L-S1,and R-Wernicke after treatment(P<0.05,P<0.01).Conclusion Heat-sensitive moxibustion could alleviate psychological craving in patients with methamphetamine addiction(withdrawal stage),possibly through complex regulatory mechanisms involving multiple brain regions.
