中药干预缺氧诱导因子信号通路治疗骨关节炎研究进展OA
Research Progress on Traditional Chinese Medicine Intervening in the Hypoxia-Inducible Factor Signaling Pathway for the Treatment of Osteoarthritis
目的 为开发骨关节炎(OA)治疗新靶点提供理论依据.方法 通过检索国内外相关文献,总结缺氧诱导因子(HIF)信号通路在OA 病理进程中的作用机制,重点分析 HIF-1α 的软骨保护功能(能量代谢调控、自噬激活、细胞外基质稳态维持、抗凋亡作用)与HIF-2α 的软骨破坏机制(分解代谢诱导、炎症放大、细胞死亡促进).梳理中药单体及复方调控 HIF 通路干预 OA 的研究,归纳其作用靶点、信号网络及生物学效应.结果 HIF-1α 通过调控糖酵解关键酶、线粒体功能因子及自噬相关分子,维持软骨细胞能量稳态与存活;同时直接激活软骨基质基因表达,抑制 Wnt/β-catenin 通路介导的基质降解.HIF-2α 则通过上调促炎及基质降解因子,驱动软骨破坏,并与核因子 κB(NF-κB)形成正反馈循环而放大炎症.中药单体可通过激活 HIF-1α/性别决定区 Y 框蛋白 9(SOX9)轴促进软骨合成代谢(淫羊藿苷、黄芩苷);通过抑制 NF-κB/HIF-2α 轴减轻炎症与基质降解(姜黄素、汉黄芩苷、补骨脂异黄酮).中药复方(如阳和汤、温经通络汤、盘龙七片)通过多靶点协同调节 HIF-1α/HIF-2α 平衡,改善软骨微环境氧化应激与能量代谢紊乱,延缓 OA 进展.结论 中药通过多途径调控 HIF 信号通路中 HIF-1α 与 HIF-2α 的活性平衡,发挥软骨保护与抗炎作用,体现了"多成分-多靶点-多通路"的协同优势.
Objective To provide a theoretical basis for the development of new therapeutic targets for osteoarthritis(OA).Methods By retrieving relevant domestic and international literature,the mechanisms of action of the hypoxia-inducible factor(HIF)signaling pathway in the pathological process of OA were summarized.The cartilage-protective functions of HIF-1α(energy metabolism regulation,autophagy activation,extracellular matrix homeostasis maintenance,and anti-apoptotic effects)and the cartilage-destructive mechanisms of HIF-2α(catabolism induction,inflammatory amplification,and promotion of cell death)were analyzed with emphasis.Studies on the regulation of the HIF pathway by traditional Chinese medicine(TCM)monomers and formulas in OA intervention were reviewed,and their targets,signaling networks,and biological effects were summarized.Results HIF-1α maintains chondrocyte energy homeostasis and survival by regulating key glycolytic enzymes,mitochondrial functional factors,and autophagy-related molecules;meanwhile it directly activates the expression of cartilage matrix genes and inhibits Wnt/β-catenin pathway-mediated matrix degradation.HIF-2α drives cartilage destruction by up-regulating pro-inflammatory and matrix-degrading factors,and forms a positive feedback loop with nuclear factor κB(NF-κB)to amplify inflammation.TCM monomers can promote cartilage anabolism by activating the HIF-1α/sex-determining region Y box protein 9(SOX9)axis(such as icariin,baicalin),and alleviate inflammation and matrix degradation by inhibiting the NF-κB/HIF-2α axis(such as curcumin,wogonoside,corylin).TCM formulas(such as Yanghe Decoction,Wenjing Tongluo Decoction,Panalongqi Tablets)can synergistically regulate the balance between HIF-1α and HIF-2α through multiple targets,improve oxidative stress and energy metabolism disorders in the cartilage microenvironment,thereby delaying OA progression.Conclusion TCM exerts cartilage-protective and anti-inflammatory effects by regulating the activity balance between HIF-1α and HIF-2α in the HIF signaling pathway through multiple approaches,reflecting the synergistic advantage of ″multi-component,multi-target,multi-pathway″.
