目的:探讨三维动态增强磁共振血管成像(3D CE-MRA)技术对胸腹部大血管病变的诊断价值.方法:回顾性分析2016年5月至2025年6月中国人民解放军空军特色医学中心收治的32例胸腹部大血管病变患者的影像与临床资料.所有患者均先行常规MRI平扫,再采用小剂量团注测试(test bolus)技术经静脉团注2 ml对比剂钆贝葡胺,测定靶血管峰值时间,以此确定个体化的3D CE-MRA扫描延迟时间.采用3D T1加权快速扰相小角度梯度回波(3D FLASH)序列,于对比剂注射前采集蒙片作为减影基础图像,再按既定延迟时间行动态增强扫描,获得动脉期和静脉期薄层原始图像,并自动完成蒙片减影.由两名影像科主治医师对原始图像及最大密度投影(MIP)重组图像进行分析,评估靶血管形态和病变特征.结果:所有患者获得的3 D CE-MRA动脉期及静脉期图像质量均能清晰显示主动脉主干及其2~3级分支.32例患者共检出阳性病变31例(占96.9%),其中腹主动脉瘤15例(占46.9%)、肾动脉狭窄6例(占18.8%)、大动脉炎6例(占18.8%)、动脉粥样硬化3例(占9.38%)及髂动脉瘤1例(占3.13%).结论:3D CE-MRA技术能够无创、清晰地显示胸腹部大血管的解剖细节和病变特征,对于胸腹部大血管病变的诊断和术前评估具有重要临床价值.

Objective:To investigate diagnostic value of three-dimension contrast-enhanced magnetic resonance angiography(3D CE-MRA)in lesions of thoracic and abdominally great vessels.Methods:A retrospective analysis was performed on imaging and clinical data of 32 patients with lesion of thoracic and abdominally great vessels who admitted to Air Force Medical Center,PLA.during May 2016 and June 2025.All patients initially underwent conventional plain scan of magnetic resonance imaging(MRI).Subsequently,the Test Bolus technique with small dose was adopted to inject 2 ml of gadobenate dimeglumine via vein,and the time to peak of target blood vessel was tested,which was used to determine the delayed time of individual 3D CE-MRA scan.Three-dimensional T1-weighted fast low-angle shot(3D FLASH)sequence was used in this study,and the mask was collected as basic images of subtraction before injected contrast agent.Then,the dynamic enhanced scan was implemented according to predetermined delay time,so as to obtain thin-slice original images at arterial and venous phases,and automatically complete subtraction for mask.Two attending physicians at department of imaging were selected to analyze the original images and reconstructed images of the maximum intensity projection(MIP),so as to assess the morphology of the target vessels,and the lesion characteristics.Results:The obtained images of 3D CE-MRA for all patients at arterial and venous phases can clearly display main stem,and branches of 2-3 level of that of aorta.A total of 31 cases with positive lesion(96.9%)were found in 32 patients,which included 15 cases of abdominally aortic aneurysm(46.9%),6 cases of renal artery stenosis(18.8%),6 cases of Takayasu arteritis(18.8%),3 cases of atherosclerosis(9.38%),and 1 case of iliac artery aneurysm(3.13%).Conclusion:3D CE-MRA technique can non-invasively and clearly display the anatomical details and pathological features of thoracic and abdominally great vessels,which has importantly clinical value in diagnosing and preoperative assessing the lesions of thoracic and abdominally great vessels.
