三维振动排痰机的工作特性及在脊柱外科患者术后肺部并发症的应用效果评价OA
Evaluation of the working characteristics of 3D-vibration expectoration machine and the application effect of that on postoperative pulmonary complications of patients undergoing spinal surgery
目的:分析三维振动排痰机的工作特性及其在脊柱外科患者手术后肺部并发症的应用效果.方法:基于人工拍背排痰原理,设计由主机、手持治疗头、靠垫治疗头以及控制软件(V1版本)构成,通过产生特定频率、振幅和方向的振动波的医疗设备,实现对脊柱外科患者的排痰应用.回顾性收集2024年10月至12月眉山市人民医院收治的62例脊柱外科手术患者的临床资料,按区组随机化分组法,将受试者按照1∶1的比例分为对照组和观察组,每组31例.对照组予以常规方法排痰,观察组采用三维振动排痰机进行排痰处理,治疗结束后比较两组患者的术后肺部并发症、呼吸困难评分、生活质量、肺功能各项指标情况.结果:术后出现肺部并发症发生率对照组25.80%(8/31),观察组6.45%(2/31),观察组并发症发生率显著低于对照组,差异有统计学意义(x2=4.29,P<0.05);两组患者治疗前的呼吸困难评分差异无统计学意义(P>0.05),治疗后观察组患者的呼吸困难评分为(2.01±0.23)分,显著低于对照组,差异有统计学意义(t=22.422,P<0.05);治疗后,观察组患者的欧洲五维健康量表(E5Q)评分、总体变化量表(GRCS)评分分别是(23.65±1.96)分和(4.65±0.32)分,均显著高于对照组,差异均有统计学意义(t=8.488、21.177,P<0.05);治疗前两组患者的用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC指标差异无统计学意义(P>0.05).治疗后观察组的FVC、FEV1、FEV1/FVC指标分别是(3.65±0.12)L、(1.95±0.10)L和(59.36±1.05)%,均显著高于对照组,差异均有统计学意义(t=19.023、10.254、11.254,P<0.05).结论:三维振动排痰机的研发和实践应用,可显著降低脊柱外科患者术后肺部并发症发生率,减少呼吸困难风险,提升患者生活质量,改善肺功能指标.
Objective:To analyze application effect of three-dimension(3D)-vibration expectoration machine in postoperative pulmonary complications of patients who underwent spinal surgery.Methods:Based on the principle of manual expectoration through pats back,a device consisting of main unit,handheld treatment head,treatment head of cushion type,and control software(version V1)was designed,which can realize expectoration for patients who undergo spinal surgery through generates vibration waves with specific frequency,amplitude and direction.A total of 62 patients who underwent spinal surgery at Meishan People's Hospital from October to December 2024 were retrospectively selected as research subjects.Using block randomization grouping method,the subjects were divided into the control group and the observation group as a ratio of 1 to 1,with 31 cases in each group.The control group received conventional expectoration method,while the observation group adopted 3D-vibration expectoration machine to implement expectoration.After treatment,the postoperative pulmonary complications,scores of respiratory difficulty,quality of life,and various indicators of lung function between the two groups were compared.Results:The incidences of postoperative pulmonary complications of control group and observation group were respectively 25.80%(8/31)and 6.45%(2/31),and the incidence of observation was significantly lower than that of control group(x2=4.29,P<0.05).There was no statistically significant difference in the score of respiratory difficulty between the two groups before treatment(P>0.05).After treatment,the score of respiratory difficulty(2.01±0.23)of observation group was significantly lower than that of control group,and the difference was statistically significant(t=22.422,P<0.05).After treatment,the score of Euro Qol five-dimensional questionnaire(E5Q),and the score of global rating of change scales(GRCS)of observation group were respectively(23.65±1.96)and(4.65±0.32),which were significantly higher than those of control group(t=8.488,21.177,P<0.05).Before treatment,there were not significant differences in the forced vital capacity(FVC),first-second forced expiratory volume(FEV1),and the ratio FEV1 to FVC(FEV1/FVC)between the two groups(P>0.05).After treatment,the FVC,FEV1,and FEV1/FVC of observation group were respectively(3.65±0.12)L,(1.95±0.10)L,and(59.36±.105)%,which were significantly higher than those of control group,and the differences were statistically significant(t=19.023,10.254,11.254,P<0.05).Conclusion:The development and practical application of the 3D-vibration expectoration machine can significantly decrease the incidence of postoperative pulmonary complications of patients who undergo spinal surgery,and reduce the risk of respiratory difficulty of patients,and enhance the quality of life of patients,and improve the indicators of lung functions.
