目的 脑微出血(CMBs)发病隐匿且机制复杂,该研究旨在通过构建不同位置的脑静脉循环障碍模型,探究脑静脉循环障碍和CMBs之间的因果关系,并评估不同磁共振成像(MRI)功能序列对静脉源性CMBs的检测效能.方法 选取20只C57BL/6J小鼠,通过选择性结扎颈内、颈外静脉,建立四组不同位置的脑静脉循环障碍小鼠模型(每组n=5),分别为双侧颈内外静脉结扎(JVL)、双侧颈外静脉结扎(EJV)、双侧颈内静脉结扎(IJV)和对照(Control)组.采用MRI多模态功能成像及苏木素-伊红(HE)病理学方法进行综合评估.结果 MRI多模态功能成像显示,仅JVL组小鼠在SWI序列中检测到CMBs,其余各组未见明显异常.病理结果显示,双侧IJV组和JVL组小鼠均出现CMBs,JVL组CMBs数量更多、面积更大(P<0.05).Spearman相关性分析表明,单张切片CMBs数量(r=0.82,P<0.01)、CMBs面积(r=0.97,P<0.001)与SWI序列检出的CMBs数量均呈现出正相关.结论 脑静脉循环障碍会诱发CMBs,且不同部位的循环障碍对CMBs的致病性存在差异.针对静脉源性CMBs,磁敏感加权成像相较于T1WI、T2WI、DWI成像手段具备更高的敏感性.

[Objective]Cerebral microbleeds(CMBs)are insidious in onset and have a complex mechanism of occurrence.Therefore,this study aims to explore whether there is a causal relationship between them and CMBs by preparing models of venous circulation disorders at different positions,and to evaluate the detection efficiency of different MRI functional sequences for venous CMBs.[Methods]Twenty C57BL/6J mice were selected,and the internal and external jugular veins were selectively ligated to prepare four groups of mouse models of cerebral venous circulation disorders at different locations(n=5),namely the bilateral internal and external jugular vein ligation(JVL),bilateral external jugular vein ligation(EJV),bilateral internal jugular vein ligation(IJV),and control groups.Subsequently,MRI multimodal functional imaging,and pathological HE staining were carried out for comprehensive evaluation.[Results]Magnetic resonance multimodal functional imaging showed that CMBs appeared in the SWI sequence in the JVL group of mice,and there were no abnormalities in the brains of mice in the other groups.According to the pathological results,CMBs occurred in both the bilateral IJV group and the JVL group of mice,and the number and area of CMBs in the latter were larger(P<0.05).Spearman correlation analysis showed that the number of CMBs in a single slice(r=0.82,P<0.01)and the area of CMBs(r=0.97,P<0.001)were positively correlated with the number of CMBs detected by SWI.[Conclusion]Cerebral venous circulation disorders can lead to the occurrence of CMBs,and circulation disorders in different parts have different pathogenicity for CMBs.For venous origin CMBs,susceptibility weighted imaging has higher sensitivity compared with T1WI,T2WI,and DWI imaging methods.
