目的 探讨血清长链非编码RNA(lncRNA)SNHG14和miR-150-5p水平对重症肺炎患者预后的预测价值.方法 选取2022年6月至2025年1月本院收治的125例重症肺炎患者,根据28 d预后情况分为死亡组(n=53)与生存组(n=72).采用实时定量PCR检测患者入院时血清lncRNA SNHG14及miR-150-5p表达水平,同时收集入组患者基线临床资料.采用多因素Cox回归分析识别影响重症肺炎患者预后的因素.采用受试者操作特征曲线评估血清lncRNA SNHG14、miR-150-5p对重症肺炎患者预后的预测价值.采用Pearson相关分析血清lncRNA SNHG14和miR-150-5p的相关性.结果 死亡组患者血清lncRNA SNHG14水平、序贯器官衰竭评分(SOFA)、急性生理与慢性健康评分系统Ⅱ(APACHE Ⅱ)评分及行有创机械通气方式的患者比例显著高于生存组,血清miR-150-5p水平显著低于生存组(P<0.05).多因素Cox回归分析结果显示,机械通气方式、SOFA评分、APACHE Ⅱ评分及lncRNA SNHG14、miR-150-5p水平均为影响重症肺炎患者预后的因素(P<0.05).血清lncRNA SNHG14、miR-150-5p及其联合预测重症肺炎预后的曲线下面积分别为0.780、0.810、0.876,联合组预测效能显著高于单一指标(ZlncRNA SNHG14=2.836,P=0.005;ZmiR-150-5p=2.130,P=0.033).重症肺炎患者血清lncRNA SNHG14与miR-150-5p呈负相关(r=-0.418,P<0.05),且二者之间有靶向结合位点.结论 重症肺炎患者血清中lncRNA SNHG14高表达、miR-150-5p低表达,二者联合检测可为预测重症肺炎患者的预后提供有效参考.

Objective To evaluate the predictive value of serum long non-coding RNA(lncRNA)SNHG14 and microRNA(miR)-150-5p for the prognosis of patients with severe pneumonia.Methods This observational cohort study included 125 patients with severe pneumonia who were admitted to our hospital between June 2022 and January 2025.The patients were classified into two groups based on their 28-day prognosis as the death group(n=53)and the survival group(n=72).Real-time quantitative PCR(qRT-PCR)technology was used to measure serum lncRNA SNHG14 and miR-150-5p levels in patients upon admission.Baseline clinical data of enrolled patients were also collected.Cox regression was employed to identify factors influencing the prognosis of patients with severe pneumonia.Receiver operating characteristic(ROC)curves were constructed to evaluate the predictive value of serum lncRNA SNHG14 and miR-150-5p in the prognosis of patients with severe pneumonia.Pearson's method was used to analyze the correlation between serum lncRNA SNHG14 and miR-150-5p.Results The death group had significantly higher serum lncRNA SNHG14 levels,a higher Sequential Organ Failure Assessment(SOFA)score,Acute Physiology And Chronic Health EvaluationⅡ(APACHEⅡ)score,proportion of invasive mechanical ventilation,and conspicuously lower serum miR-150-5p levels than the survival group(P<0.05).The multivariate Cox regression indi-cated that the mechanical ventilation mode,SOFA score,APACHEⅡscore,lncRNA SNHG14,and miR-150-5p were all factors influ-encing the prognosis of patients with severe pneumonia(P<0.05).The area under the curve(AUC)values of serum lncRNA SNHG14,miR-150-5p,and their combination for predicting the prognosis of severe pneumonia were 0.780,0.810,and 0.876,respectively.The com-bination group had significantly higher predictive power than the single indicator group(ZlncRNA SNHG14=2.836,P=0.005;ZmiR-150-5p=2.130,P=0.033).The serum lncRNA SNHG14 levels in patients with severe pneumonia were negatively correlated with miR-150-5p(r=-0.418,P<0.05),indicating a potential targeted binding site between the two.Conclusion Serum lncRNA SNHG14 was highly expressed,and miR-150-5p was expressed at low levels in patients with severe pneumonia.Therefore,the combined detection of these two can provide an effective reference for predicting the prognosis of patients with severe pneumonia.
