目的 分析ROP自然消退和进展情况相关因素,了解影响ROP病变进展的因素.方法 回顾性分析168例ROP患儿,据预后情况分为进展组和消退组,对两组相关因素进行统计学分析:包括(1)早产儿一般情况:出生孕周、出生体重、吸氧时间、机械通气等;(2)早产儿实验室检查指标:血小板、血红蛋白;(3)母亲实验室检查指标及妊娠期合并症:血红蛋白、妊娠期糖尿病、妊娠期高血压疾病等.结果 168例ROP患儿中,43例(25.60%)ROP进展至重度需手术治疗,125例(74.40%)患儿病变自然消退.168例ROP进展单因素分析显示与出生孕周、出生体重、吸氧天数、机械通气天数、发生感染、合并败血症、输血、出生后4周内血小板平均浓度相关显著,差异具有统计学意义(P<0.05),多因素Logistic回归分析显示,吸氧天数是ROP病变进展的危险因素(OR>1,P<0.001).结论 吸氧天数是ROP进展的独立危险因素,吸氧天数越多,R0P进展风险越高.对早产儿及ROP患儿应严格把握用氧时间,有效阻止ROP病变进展,降低发生重症ROP的危险性.

Objective To analyze the factors related to the natural regression and progression of Retinopathy of Pre-maturity(ROP),and to identify the elements influencing the progression of ROP lesions.Methods A retrospective analy-sis was conducted on 168 infants with ROP.The infants were divided into a progression group and a regression group based on their prognosis.A statistical analysis of relevant factors from both groups was performed,including:(1)General neo-natal characteristics:gestational age,birth weight,duration of oxygen therapy,and mechanical ventilation;(2)Neo-natal laboratory indicators:platelet count and hemoglobin level;(3)Maternal laboratory indicators and pregnancy com-plications:hemoglobin level,gestational diabetes,and hypertensive disorders of pregnancy.Results Among the 168 infants with ROP,43 cases(25.60%)progressed to a severe stage requiring surgical intervention,while 125 cases(74.40%)experienced spontaneous regression of lesions.Univariate analysis of the 168 ROP cases revealed statistically significant associations between ROP progression and the following factors:gestational age,birth weight,days of oxygen supplementation,days of mechanical ventilation,occurrence of infection,presence of sepsis,blood transfusion,and the mean platelet concentration within the first 4 weeks after birth(P<0.05).Multivariate logistic regression analysis identi-fied the duration of oxygen therapy as an independent risk factor for ROP progression(OR>1,P<0.001).Conclusions The duration of oxygen exposure is an independent risk factor for the progression of ROP.A longer duration of oxygen thera-py is associated with a higher risk of ROP progression.Strict management of oxygen therapy duration in premature infants and those with ROP is essential to effectively prevent lesion progression and reduce the risk of severe ROP.
