学龄期儿童配戴多元透镜分区离焦镜片后眼轴和屈光度变化的对照研究OA
A comparative study on axial length and refractive error changes in school-age children wearing diversified seg-mental defocus optimization lens
目的 探讨配戴多元透镜分区离焦镜片与使用传统矫正方式在不同屈光状态学龄期儿童中的眼轴和屈光度变化差异.方法 前瞻性非随机对照研究.选取等效球镜度-2.00~+2.00D学龄期儿童,依受试儿童及家长意愿进入空白对照组(生理性远视不戴镜或低度近视儿童配戴单光眼镜)或试验组(配戴多元透镜分区离焦镜片 Diversified Segmental Defocus Optimization,DSDO).按散瞳检影验光的等效球镜度(Spherical equivalent,SE)结果分为:(1)低度远视组:+0.75D<SE≤+2.00D;(2)近视前期组:-0.50D<SE≤+0.75D;(3)低度近视组:-2.00D≤SE≤-0.50D.随访时间为入组后6个月和12个月.观察指标为:眼轴长度(axial length,AL)、SE、脉络膜厚度(choroidal thickness,CHT)和脉络膜血管指数(choroidal vascularity index,CVI).结果 共113例完成了12个月期的随访(失访率3.4%).试验组各亚组眼轴增长值低于对照组:低度远视组0.12±0.11mm vs 0.21±0.09mm(P=0.0003,Cohen's d=0.90);近视前期组 0.18±0.19mm vs 0.38±0.18mm(P<0.0001,d=1.08);低度近视组 0.22±0.22mm vs 0.51±0.20mm(P<0.0001,d=1.36).SE 变化趋势与眼轴变化相似.CHT在6个月时试验组中近视前期和低度近视亚组所有分区均较对照组增厚,显示组间差异;12个月时近视前期组大部分分区保持差异,低度近视组则所有分区依然保持差异.CVI变化主要见于低度远视组.结论 配戴多元透镜分区离焦镜片DSDO的儿童眼轴增长和近视进展较对照组减慢.
Objective To investigate the differences in axial length and refractive error changes between school-age children wearing a Diversified Segmental Defocus Optimization(DSDO)lens and those using conventional optical correc-tion,across different baseline refractive groups.Methods This prospective non-randomized controlled study enrolled school-age children with a spherical equivalent(SE)ranging from-2.00D to+2.00D.Based on participant and parental preference,children were assigned to either the control group(physiological hyperopes without correction or low myopes wearing single-vision spectacles)or the experimental group(wearing the DSDO lens).According to cycloplegic retinos-copy results,participants were stratified into three refractive subgroups:1)Low hyperopia:+0.75D<SE≤+2.00D;2)Pre-myopia:-0.50D<SE≤+0.75D;3)Low myopia:-2.00D≤SE≤-0.50D.Follow-up examinations were con-ducted at 6 and 12 months after enrollment.Primary outcome measures included axial length(AL),SE,choroidal thick-ness(CHT),and choroidal vascularity index(CVI).Results A total of 113 participants completed the 12-month follow-up(attrition rate:3.4%).Axial elongation was significantly lower in all experimental subgroups compared to their respective control subgroups:low hyperopia(0.12±0.11 mm vs.0.21±0.09 mm;P=0.0003;Cohen's d=0.90),pre-myopia(0.18±0.19 mm vs.0.38±0.18 mm;P<0.0001;d=1.08),and low myopia(0.22±0.22 mm vs.0.51±0.20 mm;P<0.0001;d=1.36).Changes in SE followed a similar trend to AL.At the 6-month follow-up,CHT in all measured sectors was significantly thicker in the pre-myopia and low myopia experimental subgroups compared to controls.At 12 months,these differences persisted in most sectors for the pre-myopia subgroup and in all sectors for the low myopia subgroup.Significant CVI changes were primarily observed in the low hyperopia subgroup.Conclusions Wearing DSDO lenses was associated with slower axial elongation and myopia progression compared with conventional correction.
