机器人辅助肝切除在治疗巨大肝血管瘤方面具有微创、精准、可控的优势,尤其在高风险血管解剖和深部肿瘤定位中表现突出.肝血管瘤因其血供丰富、体积巨大、位置复杂,手术过程中常面临许多技术挑战,包括术中大出血、肝静脉或下腔静脉损伤、肝功能保留等问题.术前借助三维可视化技术获取精确的解剖信息,指导术前规划.术中选择合适的体位与穿刺孔布局,综合运用供瘤血管阻断、肝门阻断等技术有效控制出血,根据肿瘤位置、大小、与主要血管关系等情况合理选择剜除术或解剖性切除,结合手术路径的改良和术中吲哚菁绿(ICG)导航,争取在最大限度保留正常肝实质的同时安全、精准地切除肝血管瘤.

Robotic-assisted hepatectomy has the advantages of being minimally invasive,precise,and controllable in the treatment of giant hepatic hemangiomas,performing particularly well in high-risk vascular anatomy and deep tumor localization.Due to their rich blood supply,massive size,and complex locations,hepatic hemangiomas often pose many technical challenges during surgery,including massive intraoperative bleeding,injury to the hepatic vein or inferior vena cava,and preservation of liver function.Preoperatively,three-dimensional visualization technology is used to obtain precise anatomical information to guide preoperative planning.Intraoperatively,appropriate body positioning and trocar layout are selected.Techniques such as feeding vessel occlusion and hepatic inflow occlusion are comprehensively utilized to effectively control bleeding.Based on the tumor's location,size,and relationship with major blood vessels,enucleation or anatomical resection is reasonably selected.Combined with the improvement of the surgical approach and intraoperative indocyanine green(ICG)navigation,efforts are made to safely and precisely resect the hepatic hemangioma while maximizing the preservation of normal liver parenchyma.
