肝血管瘤外科手术时机和适应证选择再认识OA

肝血管瘤是最常见的肝脏良性肿瘤,常在体检中发现,多数病灶生长缓慢;少数可持续增大并进展为巨大肝血管瘤,导致腹胀、食欲下降等症状.目前国内外尚缺乏高等级循证医学指南,不同中心对外科手术适应证把握存在差异.随着微创外科、术中可视化技术及人工智能辅助决策的发展,肝血管瘤诊断准确性进一步提高,围手术期创伤与风险持续下降,围绕手术时机与适应证选择的争议随之加深:是以较低手术代价提前干预潜在进展病灶,还是以严密随访为主并在症状出现后再行手术以减少过度治疗.现有证据与临床实践共同提示,手术决策应以病人症状、病灶生长趋势、解剖部位、技术可及性及预期获益—风险平衡为核心,强调个体化评估与动态再评估.在精准肝切除与微创技术背景下,建立更具操作性的分层指征体系,有助于优化治疗时机选择并提升总体获益.

Hepatic hemangioma is the most common benign liver tumor and is often detected during routine physical examination.Most lesions grow slowly,whereas a minority may enlarge progressively to giant hemangioma and cause symptoms such as abdominal distension and poor appetite.To date,high-level evidence-based guidelines are still lacking,leading to substantial inter-center variation in surgical indications.With advances in minimally invasive surgery,intraoperative visualization,and artificial intelligence-assisted decision-making,diagnostic accuracy has improved and perioperative trauma and risk have declined,which has intensified the debate on optimal timing and indications for surgery.The key controversy is whether to intervene earlier at a relatively low procedural cost to eliminate potential growth risk,or to prioritize close surveillance and operate after symptom development to avoid overtreatment.Current evidence and clinical experience support an individualized strategy based on symptoms,growth dynamics,anatomical location,technical feasibility,and expected benefit-risk balance,with dynamic reassessment over time.In the context of precision liver resection and minimally invasive surgery,a more practical risk-stratified indication framework is needed to optimize surgical timing and overall outcomes.
