Chronic respiratory diseases are characterized by long-term course,being progressive and recurrent acute exacerbations.Focusing only on acute phase treatment is far from being sufficient,and standardized whole-course management is crucial for improving patient prognosis.Current pulmonary rehabilitation management has problems of being phased and fragmented,lacking effective connection among stages,and distribution of professional resources is uneven,so it is difficult to meet the continuous rehabilitation needs of patients.The essential content of each stage in constructing a closed-loop management system for pulmonary rehabilitation from the intensive care unit(ICU)to home includes:in the ICU stage,the focus is on early warning and preventive rehabilitation,specific start-up indicators such as stable vital signs are clarified,and multi-system rehabilitation interventions and digital monitoring are integrated;the aim in the transition period is to build a bridge of continuous nursing care through high-dependence unit(HDU)and specialized rehabilitation institutions,strengthening multidisciplinary collaboration and survival capacity guarantee;in the home rehabilitation stage,digital tools are employed to resolve the contradiction between professionalism and accessibility,improving patient compliance through behavioral interventions.With multidisciplinary collaboration,data-driven and quality control as the core pillars,this system realizes precise whole-course management through digital tools,which can effectively delay disease progression,reduce readmission rate and improve patients' quality of life.The clinical practice of our team shows that the closed-loop management model can significantly improve the exercise tolerance and self-management ability of patients with chronic obstructive pulmonary disease(COPD),interstitial lung disease,etc.,providing a promotable practical path for pulmonary rehabilitation.
