Epidemiological characteristics and antimicrobial resistance gene profiles of ESBL-producing Klebsiella pneumoniae from chickens in Xinjiang

为了解新疆地区蛋鸡养殖场中产超广谱β-内酰胺酶(Extended-spectrum β-lactamases,ESBL)肺炎克雷伯菌的流行特征及耐药基因携带情况,本研究从新疆5个地区6个蛋鸡养殖场采集鸡粪便样本进行肺炎克雷伯菌的分离鉴定,并采用CLSI推荐的ESBL鉴定产ESBL菌株,通过微量肉汤稀释法对其进行药敏试验;选取不同地区的产ESBL菌株进行全基因组测序,分析菌株所携带的耐药基因及质粒复制子类型,并基于核心基因组构建系统发育树.结果显示:1)1 115份鸡粪便样品中共鉴定出产ESBL肺炎克雷伯菌39株,检出率为3.5%.2)所有产ESBL分离株对四环素、头孢噻肟和环丙沙星的耐药率分别为89.7%、87.2%和84.6%,对美罗培南均敏感.多重耐药菌株占比97.4%.3)35株不同地区产ESBL菌株中共检测到12类66种耐药基因,blaSHV、oqxA和oqxB的携带率最高(均为97.1%).共发现14株携带介导替加环素耐药的外排泵基因tmexCD-TOprJ,主要分布在ST37(11/14)中.4)共检测到12种质粒复制子,82.9%(29/35)的菌株携带至少1种复制子类型,其中IncFIB(K)最为常见(72.4%).5)35株菌共鉴定出12种ST型,整体呈现较高的遗传多样性.未发现跨区域的克隆传播,但在塔城鸡场中检测到ST37、ST307、ST5116与ST200等ST型的紧密聚类.综上,新疆蛋鸡养殖场中产ESBL肺炎克雷伯菌检出率较低,但菌株耐药谱广、携带多种耐药基因与质粒复制子,且部分菌株存在克隆传播风险,提示需加强区域性耐药菌监测、合理使用抗菌药物以降低耐药菌传播.

To investigate the epidemiological characteristics and antimicrobial resistance genes of extended-spectrum β-lactamase(ESBL)-producing Klebsiella pneumoniae(K.pneumoniae)in layer farms in Xinjiang,fecal samples were collected from six layer farms across five regions.K.pneumoniae isolates were obtained and identified,and ESBL-producing strains were screened using the CLSI-recommended confirmatory test.Antimicrobial susceptibility testing was performed using the broth microdilution method.Whole-genome sequencing was conducted for ESBL-producing isolates from different regions to analyze their resistance genes and plasmid replicon types.A phylogenetic tree based on the core genome was constructed.The results showed that:1)A total of 39 strains of ESBL-producing Klebsiella pneumoniae were identified in 1 155 chickens fecal samples,with a detection rate of 3.5%.2)The resistance rates of all ESBL isolates to tetracycline,cefotaxime,and ciprofloxacin were 89.7%,87.2%and 84.6%,respectively,and they were all susceptible to meropenem.The multi-drug resistant rate was 97.4%.3)Among the 35 ESBL-producing strains from different regions,66 resistance genes across 12 categories were detected.The most prevalent genes were blaSHV,oqxA,and oqxB(all 97.1%).A total of 14 strains were found to carry the tmexCD-TOprJ efflux pump gene conferring tigecycline resistance,mainly distributed in ST37(11/14).4)Twelve plasmid replicon types were identified;82.9%(29/35)of isolates harbored at least one replicon,with IncFIB(K)being the most common(72.4%).5)Twelve sequence types(STs)were identified among the 35 strains,reflecting high genetic diversity.No evidence of cross-regional clonal dissemination was found;However,tight clustering of ST37,ST307,ST5116,and ST200 was observed within the Tacheng farm.In conclusion,although the detection rate of ESBL-producing K.pneumoniae in Xinjiang layer farms was relatively low,the isolates exhibited broad resistance spectra,carried multiple resistance genes and plasmid replicons,and showed evidence of clonal transmission in certain farms.These results highlight the need for strengthened regional monitoring and control measures for antimicrobial-resistant bacteria.
