我国基层医生组织管理和服务协调能力现状研究OA
Status quo of organizational management and service coordination capacity of primary care physicians in China
目的 了解我国基层医生组织管理与服务协调能力现状,探讨差异原因与提升策略.方法 基于课题组2020年7-8月在四省一市开展的基层医生服务能力调查结果,采用SPSS 27.0整理分析数据,运用x2检验,多组间两两比较采用Bonferroni法调整检验水准,比较不同地区、机构层级基层医生组织管理与服务协调能力具备情况的差异.结果 共调查8 469名基层医生.明确分工协作、开展团队服务、沟通交流、政策解读与宣传、合理利用社区资源这五项能力具备率分别为97.1%、96.3%、97.3%、93.5%、89.7%.农村地区基层医生各项能力具备率均高于城市地区(P<0.05),中部地区各项能力具备率均高于东、西部地区(P<0.017);社区卫生服务站、村卫生室医生各项能力具备率都高于社区卫生服务中心、乡镇卫生院(P<0.05).结论 我国基层医生团队中高素质人才缺乏,政策解读与宣传、合理利用社区资源能力仍有待加强.西部地区、城市地区和较高层级基层机构医生的组织管理与服务协调能力具备率整体较低.建议构建跨部门合作机制、强化专项培训并优化其形式,设计评价指标纳入绩效考核,以全面提升基层医生组织管理与服务协调能力.
Objective To explore the status quo of the organizational management and service coordination capacity of primary care physicians in China,and to analyze the reasons for differences and improvement strate-gies.Methods Based on the survey results conducted by the research group from July to August 2020 in four provinces and one municipality on the capacity of primary care physicians,SPSS 27.0 was used for data collation and analysis.The Chi-square test was employed,and the Bonferroni method was used to adjust the test signifi-cance level for pairwise comparisons among multiple groups to compare the differences in the organizational man-agement and service coordination skills of primary care physicians at the levels of institutions and regions.Results Totally 8 469 primary care physicians were surveyed.The possession rates of five capabilities(work and coordi-nation,team service,communication,policy interpretation and publicity,and rational use of community re-sources)were 97.1%,96.3%,97.3%,93.5%,and 89.7%,respectively.Primary care physicians in rural areas demonstrated higher possession rates across five capabilities compared to those in urban areas(P<0.05).The rates of physicians in the central region was higher than those in the eastern and western regions(P<0.017).Primary care physicians in community health service stations and village clinics had higher rates of all capabilities than those in community health service centers and township health centers(P<0.05).Conclusions There is a shortage of high-quality personnel in China's primary care physician team.The abilities of policy interpretation and publicity,and rational use of community resources still need to be strengthened.The overall rate of primary care physicians' organizational management and service coordination capacity in western regions,urban areas,and higher-level primary care institutions is relatively low.It is necessary to establish a cross-departmental cooperation mechanism,strengthen special training and optimize its formats,design evaluation indicators to incorporate into performance evaluation system,comprehensively enhancing primary care physicians' organizational management and service coordination capacity.
