目的:探讨初诊多发性骨髓瘤(MM)患者外周血中CD4+T淋巴细胞、CD8+T淋巴细胞及Treg上的负性共刺激分子CD276表达及其临床意义.方法:收集2023年9月至2024年10月在兰州大学第二医院住院的34例初诊MM患者作为实验组,16例健康体检者作为对照组.流式细胞术检测外周血中CD4+T细胞、CD8+T细胞和Treg CD276表达,并分析其与MM患者临床特征及临床检验指标的相关性.结果:初诊MM患者外周血中CD4+T细胞、CD8+T细胞和Treg CD276表达显著高于健康对照组(P<0.05).进一步分析显示,CD4+T细胞和Treg CD276表达与MM的R-ISS分期相关.R-ISS分期Ⅲ期患者CD4+T细胞CD276表达显著高于Ⅰ~Ⅱ期患者(P<0.05),Ⅲ期患者Treg CD276表达亦显著高于Ⅰ~Ⅱ期患者(P<0.05),且高危MM患者Treg CD276表达显著高于标危患者(P<0.05).此 外,CD4+T细胞和Treg CD276表达与细胞因子IL-4呈负相关(r=-0.376、-0.402,P<0.05),CD8+T细胞和Treg CD276表达与IL-8呈负相关(r=-0.384、-0.365,P<0.05).CD4+T细胞、CD8+T细胞和Treg CD276表达与补体3(C3)、IgA、胆固醇(CHOL)及B细胞比例等临床检验指标显著相关.结论:初诊MM患者CD4+T细胞、CD8+T细胞及Treg CD276表达显著升高,并与R-ISS分期和疾病危险度分层等临床指标相关.CD276异常表达可能参与MM的发生发展,具有潜在的预后评估价值及治疗靶点意义.

Objective:To investigate expression and clinical significance of negative costimulatory molecule CD276 on periph-eral blood CD4+T lymphocytes,CD8+T lymphocytes and Treg in newly diagnosed multiple myeloma(MM)patients.Methods:Periph-eral blood samples were collected from 34 MM patients hospitalized at the Second Hospital of Lanzhou University between September 2023 and October 2024 as experimental group,and from 16 healthy individuals as control group.Flow cytometry was used to measure CD276 expression on CD4+T cells,CD8+T cells and Treg,and correlations between CD276 expression and clinical characteristics or laboratory indices in MM patients were analyzed.Results:CD276 expression on CD4+T cells,CD8+T cells and Treg in peripheral blood of newly diagnosed MM patients were significantly higher than healthy control group(P<0.05).Further analysis showed that CD276 expression on CD4+T cells and Treg were associated with R-ISS stage of MM.Specifically,CD276 expression on CD4+T cells was significantly higher in patients with stage Ⅲ than in those with stage Ⅰ~Ⅱ(P<0.05),and CD276 expression on Treg was signifi-cantly higher in stage Ⅲ patients compared to those in stage Ⅰ~Ⅱ(P<0.05).Additionally,Treg CD276 expression was markedly higher in high-risk MM patients than standard-risk patients(P<0.05).CD276 expression on CD4+T cells and Treg were negatively cor-related with cytokine IL-4(r=-0.376,-0.402,P<0.05),while CD276 expression on CD8+T cells and Treg was negatively correlated with IL-8(r=-0.384,-0.365,P<0.05).Notably,CD276 expression on CD4+T cells,CD8+T cells and Treg was also significantly corre-lated with laboratory indices,including complement 3(C3),IgA,cholesterol(CHOL)and proportion of B cells.Conclusion:CD276 expression on CD4+T cells,CD8+T cells and Treg are significantly elevated in newly diagnosed MM patients and associates with clini-cal indicators such as R-ISS staging and disease risk stratification.Abnormal CD276 expression may contribute to pathogenesis of MM,with potential value as a prognostic marker and therapeutic target.
