目的:探讨上颌Nance弓联合横腭杆(TPA)和微种植体支抗治疗双颌前突或上颌单颌前突的临床效果.方法:回顾性分析2021年6月-2024年8月笔者医院收治的54例双颌前突或上颌单颌前突患者的临床资料,按治疗方法不同分为Nance弓+TPA组30例和联合组24例,Nance弓+TPA组采取上颌Nance弓联合TPA治疗,联合组采用Nance弓+TPA组联合微种植体支抗治疗,比较两组矫治时间,治疗前、后头影测量指标(SNA角、SNB角、ANB角,U1-NA角、U1-SN角、L1-NB角、UL-EP、LL-EP)、软组织变化指标(鼻唇角、颏沟倾角)、牙周指标(牙龈指数、牙菌斑指数、探诊深度、附着丧失)差异,以及不良反应发生情况.结果:联合组的拔牙间隙完全关闭时间、矫治时间低于Nance弓+TPA组(P<0.05);矫治后,联合组U1-NA角、U1-SN角、L1-NB角、牙菌斑指数、牙龈指数、探诊深度和附着丧失均低于Nance弓+TPA组(P<0.05),UL-EP、LL-EP距离、U1-L1角、鼻唇角、颏沟倾角均高于Nance弓+TPA组(P<0.05);联合组不良反应发生率为8.33%,低于Nance弓+TPA组(33.33%)(P<0.05).结论:上颌Nance弓联合TPA和微种植体支抗治疗双颌前突或上颌单颌前突,可缩短矫治时间,改善患者上颌前突和软组织美观度,且安全性较高.

Objective To explore the clinical effect of maxillary Nance arch combined with transverse palatal arch(TPA)and micro-implant anchorage control anchorage in the treatment of bimaxillary protrusion or monomaxillary protrusion.Methods The clinical data of 54 patients with bimaxillary protrusion or monomaxillary protrusion in the hospital were retrospectively analyzed from From June 2021 to August 2024.According to different treatment methods,they were divided into Nance arch+TPA group(30 cases),combined group(24 cases).The Nance arch+TPA group was treated with maxillary Nance arch combined with TPA,while the combined group was added with micro-anchorage nail control anchorage on the basis of the Nance arch+TPA group.The orthodontic treatment time,cephalometric indexes(SNA angle,SNB angle,ANB angle,UI-NA angle,U1-SN angle,L1-NB angle,UL-EP,LL-EP),soft tissue change indexes(nasolabial angle,mental groove inclination angle)and periodontal indexes(gingival index,plaque index,probing depth,attachment loss)before and after treatment and occurrence of adverse reactions were compared among the groups.Results The complete closure time of extraction space and the orthodontic treatment time in combined group was shorter than that in Nance arch+TPA group(P<0.05).After orthodontic treatment,the UI-NA angle,U1-SN angle,L1-NB angle,dental plaque index,gingival index,probing depth and attachment loss in combined group were lower than those in Nance arch+TPA group(P<0.05),and the UL-EP,LL-EP distance,U1-L1 angle,the nasolabial angle and mental groove inclination angle were higher than those in Nance arch+TPA group(P<0.05).The incidence of adverse reactions with 8.33%in combined group was lower than 33.33%in Nance arch+TPA group(P<0.05).Conclusion Maxillary Nance arch combined with TPA and micro-implant anchorage control anchorage can shorten the orthodontic treatment time and improve the maxillary protrusion and soft tissue changes in the treatment of bimaxillary protrusion or monomaxillary protrusion,and it has high safety.
