JAK抑制剂在慢性手部湿疹治疗中的应用及其复发的影响因素分析OA
Clinical Efficacy and Recurrence Influencing Factors of JAK Inhibitors in the Treatment of Chronic Hand Eczema
目的:探讨Janus激酶(Janus Kinase,JAK)抑制剂在慢性手部湿疹(Chronic Hand Eczema,CHE)治疗中的临床效果,并对影响疾病复发的相关因素进行分析.方法:选取2021年10月-2024年10月笔者医院收治的CHE患者156例,根据随机数字表法分为对照组和研究组,每组78例.对照组接受常规治疗,研究组接受常规治疗联合JAK抑制剂治疗.治疗持续4周,每周随访1次.比较两组的临床疗效,治疗前后手部湿疹评分系统(Jena Hand Eczema Score,JHS)评分和瘙痒视觉模拟评分(Visual Analog Scale,VAS)评分.治疗结束后,对两组对治疗效果达到有效及以上的患者进行为期6个月的随访,比较复发情况并分析复发的影响因素.结果:研究组治疗总有效率92.31%高于对照组的80.77%(P<0.05);治疗4周后,两组JHS、VAS评分均显著降低,且研究组低于对照组(P<0.05);对治疗效果达到有效及以上的患者随访6个月后,两组各有2例患者失访,最终纳入131例患者,其中57例复发(复发组),74例未复发(未复发组).单因素分析结果显示,复发组既往有过敏史、家族湿疹病史、饮酒、常规联合JAK抑制剂治疗、手部接触刺激性物质占比,基线JHS评分、VAS评分均高于无复发组,保湿剂使用频率低于无复发组(P<0.05);进一步多因素分析结果显示,既往有过敏史、治疗方式、手部接触刺激性物质、基线JHS评分和VAS评分均为CHE治疗后6个月内复发的独立影响因素(P<0.05),其中治疗方式为保护因素(OR=0.321),其余均为危险因素(OR>1).结论:JAK抑制剂治疗慢性手部湿疹效果显著,能有效缓解症状,改善瘙痒程度.既往有过敏史、治疗方式、手部接触刺激性物质、基线JHS评分、基线VAS评分均是慢性手部湿疹治疗后6个月内复发的独立影响因素,其中治疗方式为保护因素,其余均为危险因素.
Objective To evaluate the clinical efficacy of Janus kinase(JAK)inhibitors in treating chronic hand eczema(CHE)and to identify factors influencing disease recurrence.Methods A total of 156 patients with CHE admitted to the hospital from October 2021 to October 2024 were randomly assigned to a control group and a study group,each comprising 78 patients.The control group underwent conventional treatment,whereas the study group received a combination of conventional treatment and JAK inhibitor therapy.The treatment duration was 4 weeks,with weekly follow-ups.The clinical efficacy was compared between the two groups,including the Jena Hand Eczema Score(JHS)and Itching Visual Analog Scale(VAS)score before and after treatment.After completing the treatment,a 6-month follow-up was conducted to compare the recurrence rates and analyze the factors influencing recurrence.Results The study group exhibited a higher total effective rate of 92.31%compared to the control group's 80.77%(P<0.05).Following 4 weeks of treatment,both groups demonstrated a significant reduction in JHS and VAS scores,with the study group achieving lower scores than the control group(P<0.05).After 6 months of follow-up,with 2 patients lost to follow-up in each group,a total of 131 patients were included in the analysis,comprising 57 patients with recurrence(recurrence group)and 74 without recurrence(non-recurrence group).Univariate analysis revealed that the recurrence group had a higher prevalence of allergic history,family history of eczema,alcohol consumption,use of JAK inhibitors in combination with conventional therapy,and contact with hand irritants.They also had higher baseline JHS and VAS scores and a lower frequency of moisturizer use compared to the non-recurrence group(P<0.05).Multivariate analysis further indicated that allergic history,treatment modality,exposure to hand irritants,baseline JHS score,and VAS score were independent factors influencing recurrence within 6 months post-CHE treatment(P<0.05).The treatment modality acted as a protective factor(OR=0.321),while the remaining factors were identified as risk factors(OR>1).Conclusion JAK inhibitors demonstrate significant therapeutic efficacy in managing chronic hand eczema,effectively alleviating symptoms and reducing itching intensity.Allergic history,treatment approach,exposure to hand irritants,baseline JHS score,and baseline VAS score are identified as independent factors influencing the recurrence of chronic hand eczema within 6 months after treatment.The treatment approach serves as a protective factor,whereas the other factors are considered risk factors.
