关节纤维化的病因、发病机制与临床诊疗进展OA
Advances in the etiology,pathogenesis and clinical management of joint fibrosis
关节纤维化是指关节内或关节周围组织由于各种原因发生纤维过度增生,从而影响关节正常功能的一种病理状态.关节纤维化的发病机制尚不明确,可能由外伤、慢性炎症、免疫应激、代谢性疾病等多种因素引起,涉及细胞增殖、纤维化信号通路的激活及细胞外基质重塑等复杂过程.尽管临床上已提出多种治疗策略,但目前治疗效果仍不理想.本文系统综述了关节纤维化的主要病因、发病机制及现有治疗方法,并提出未来研究方向与潜在治疗策略,为关节纤维化的基础和临床研究提供思路.
Arthrofibrosis is a pathological condition characterized by excessive fibrous tissue hyperplasia occurring in or around the joint due to various etiological factors,leading to impaired normal joint function.The exact patho-genesis of arthrofibrosis remains incompletely elucidated,encompassing complex processes such as cellular prolifer-ation,activation of fibrogenic signaling pathways,and extracellular matrix remodeling.These processes may be ini-tiated by multiple inciting factors,including trauma,chronic inflammation,immune stress,and metabolic disor-ders.Despite advancements in therapeutic approaches,clinical outcomes for arthrofibrosis remain suboptimal.This review systematically synthesized the major etiological factors,pathological mechanisms,and current treatment mo-dalities for arthrofibrosis,while also delineating future research directions and potential therapeutic strategies to en-hance both scientific research and clinical management of this condition.
