肱骨头囊性变与肩袖损伤的相关性研究OA

Objective To investigate the MRI correlation between cystic degeneration of the humeral head and rotator cuff injury.Methods A retrospective analysis was conducted on the MRI of 156 patients with rotator cuff injury confirmed by surgery in Cangzhou People's Hospital from January 2023 to January 2025.Patients were divided into the cystic degeneration group(n=83)and the non-cystic degeneration group(n=73)based on the presence of cystic degeneration.The injury conditions of different tendons in the non-cystic degeneration group and the cystic degeneration group were compared.The tendon injury conditions of patients with anterior and posterior greater tuberosity of humerus,lesser tuberosity of humerus and anatomical neck cystic degeneration were compared.Results Among the 156 patients,83 patients(53.2%)had isolated supraspinatus tendon tears,42 patients(26.9%)had tears involving two tendons(supraspi-natus+infraspinatus),and 31 patients(19.9%)had tears involving all three tendons(supraspinatus+infraspinatus+subscapularis).The differences in tendon injuries between patients with cystic degeneration located in the anterior greater tuberosity,posterior greater tuberosity,and anatomical neck showed no statistical significance(P>0.05).However,The incidence of injury involving all three tendons was higher than those of supraspinatus tendon injury and supraspinatus tendon+infraspinatus tendon injury in patients with lesser tuberosity cystic degeneration,and the differences were statistically significant(P<0.05).Conclusion The incidence of combined three-tendon injury is significantly higher than that of isolated single-tendon injury in patients with lesser tuberosity cystic degeneration,suggesting that cystic degeneration of lesser tuberosity may have a specific association with more extensive,complex rotator cuff injury(involving supraspi-natus,infraspinatus and subscapularis),which has potential reference value for assessing the severity of rotator cuff injuries and formulating individualized diagnosis and treatment plans.
