身体质量指数对胰腺手术后手术部位感染影响的回顾性队列研究OA
Retrospective cohort study on the impact of body mass index on surgical site infection after pancreatic surgery
目的 探索身体质量指数(BMI)与胰腺手术后手术部位感染(SSI)风险的关系,并分析不同人群 BMI对 SSI 的影响.方法 采用回顾性队列研究,纳入 2022 年 1 月—2023 年 6 月在南京医科大学第一附属医院接受胰腺手术的患者,按 BMI 分为低体重组(BMI<18.5 kg/m2)、正常体重组(18.5 kg/m2≤BMI<24 kg/m2)、超重组(24 kg/m2≤BMI<28 kg/m2)和肥胖组(BMI≥28 kg/m2),采用 logistic 回归分析 BMI 对 SSI 的影响.结果 共纳入 1 345 例患者,胰腺手术后 SSI 发病率为 8.55%,其中低体重组、正常体重组、超重组和肥胖组 SSI 发病率分别为 3.57%、5.17%、12.36%、19.27%.Logistic 多因素分析结果显示,超重组发生 SSI 风险是正常组的 2.487 倍(95%CI:1.570~3.939,P<0.001),肥胖组发生 SSI 风险为正常组的 5.353 倍(95%CI:2.854~10.040,P<0.001).亚组分析发现,年龄≥60 岁人群中 BMI 对 SSI 风险的影响更加显著(交互项 P=0.009).结论 BMI 升高显著增加胰腺手术患者 SSI 风险,且在高龄人群中影响更为明显.建议将 BMI 纳入术前感染风险评估,结合国家"体重管理年"政策,推动个体化体重管理与精准感染防控.
Objective To explore the relationship between body mass index(BMI)and the risk of surgical site infection(SSI)after pancreatic surgery,and analyze the impact of BMI on SSI in different populations.Methods Patients who underwent pancreatic surgery at the First Affiliated Hospital with Nanjing Medical University from January 2022 to June 2023 were included for retrospective cohort study.Based on BMI,patients were divided into low weight group(BMI<18.5 kg/m2),normal weight group(18.5 kg/m2≤BMI<24 kg/m2),overweight group(24 kg/m2≤BMI<28 kg/m2),and obese group(BMI≥28 kg/m2).Impact of BMI on SSI was analyzed by logistic regression.Results A total of 1 345 patients were included in the analysis.Incidence of SSI after pancreatic surgery was 8.55%.Incidences of SSI in low weight group,normal weight group,overweight group,and obesity group were 3.57%,5.17%,12.36%,and 19.27%,respectively.Multivariate logistic analysis results showed that the risk of SSI occurrence in overweight group was 2.487 times of normal group(95%CI:1.570-3.939,P<0.001),and that in obese group was 5.353 times of normal group(95%CI:2.854-10.040,P<0.001).Subgroup analy-sis found that BMI had a more significant impact on the risk of SSI in individuals aged≥60 years old(interaction term P=0.009).Conclusion Elevated BMI significantly increases the risk of SSI in pancreatic surgery patients,and the impact is more obvious in the elderly population.It is recommended to incorporate BMI into preoperative infection risk assessment,together with the national"Weight management year"policy,to promote individualized weight management and precise infection prevention and control.
