目的 系统评价不同管理模式对降低中国医疗机构医务人员血源性职业暴露发生率的效果.方法 检索中国知网、万方、PubMed 等数据库中关于不同管理模式对医务人员职业暴露发生率影响的相关文献,检索时间为建库至 2025 年 2 月 25 日.采用纽卡斯尔-渥太华(Newcastle-Ottawa,NOS)量表评价研究质量,应用 R 软件进行 Meta 分析,Stata 软件进行网状 Meta 分析.结果 共纳入 31 篇观察性研究文献,涉及 7 种主动管理模式.网状 Meta 分析结果显示,与常规管理相比,7 种主动管理模式[4R 危机管理模式、失效模式与效应分析(FMEA)、PDCA 循环管理法、品管圈(QCC)、六西格玛法、SHEL 事故分析法及根本原因分析(RCA)]均能降低医务人员职业暴露发生风险(均 P<0.05).然而,7 种主动管理模式的相互比较差异均无统计学意义.最佳概率排序显示,FMEA 成为最佳模式的概率(75.3%)最高.护士与混合职业人员亚组中 FMEA 最优,实习护士亚组中 PDCA最优.结论 PDCA 等主动管理模式在降低职业暴露发生风险方面均优于常规管理,FMEA 可能是最佳方法.最优管理模式可能因人员类别而异,实践中需灵活选用.

Objective To systematically evaluate the effect of different management modes on reducing the inci-dence of bloodborne occupational exposure among medical staff in Chinese medical institutions.Methods Literature on the effect of different management modes on the incidence of occupational exposure among medical staff were retrieved from China National Knowledge Infrastructure(CNKI),Wanfang Data,and PubMed.Search period was from database inception to February 25,2025.The Newcastle-Ottawa scale(NOS)was used to assess the quality of the studies,R software was used for Meta-analysis and Stata software for network Meta-analysis.Results A total of 31 observational studies involving 7 active management modes were included in analysis.Network Meta-analysis results showed that compared with conventional management,7 active management modes(reduction/readiness/response/recovery[4R]crisis management mode,failure mode and effect analysis[FMEA],plan-do-check-action[PDCA]cycle management method,quality control circle[QCC],Six Sigma method,soft/hard/envi-ronment/litigant[SHEL]accident analysis method,and root cause analysis[RCA])could all reduce the risk of occupational exposure among medical staff(all P<0.05).However,there were no statistically significant diffe-rences among 7 active management modes.The probability ranking indicated that FMEA had the highest likelihood of being the optimal mode(75.3%).FMEA was optimal in the nurse and mixed professional subgroups,while PDCA was optimal in the nursing interns subgroup.Conclusion Active management modes such as PDCA are superior to conventional management in reducing the risk of occupational exposure,with FMEA being the most effective method.The optimal management mode may vary with personnel category and should be flexibly applied in practice.
