血清抗dsDNA抗体联合IL-21、BCL6、CD71 mRNA水平对早期系统性红斑狼疮的诊断价值OA
Diagnostic Value of Serum Anti-dsDNA Antibodies Combined with IL-21,BCL6,and CD71 mRNA Expression in Peripheral Blood Mononuclear Cells for Early Systemic Lupus Erythematosus
目的:探讨抗dsDNA抗体、白细胞介素-21(IL-21)、B细胞淋巴瘤因子6(BCL6)及转铁蛋白受体CD71联合检测在早期系统性红斑狼疮(systemic lupus erythematosus,SLE)中的临床诊断价值.方法:收集2021年1月-2025年6月初诊SLE患者41例(包含活动期8例、非活动期33例)与健康对照者41例的血液样本.分离受试者外周血单个核细胞(PBMCs),检测IL-21、BCL6及转铁蛋白受体CD71 mRNA的表达水平;采用免疫印迹法检测患者血清中抗dsDNA抗体的含量.采用Spearman相关分析评估各指标与SLE疾病活动指数(SLEDAI)间的关联;ROC曲线分析评估单一及联合指标对SLE的诊断效能.结果:SLE患者PBMCs中IL-21、BCL6及CD71 mRNA表达水平均高于健康对照组(P<0.05),且上述指标在活动期SLE患者中的表达水平显著高于非活动期患者(P<0.0001);Spearman 相关分析表明 IL-21、BCL6、CD71 mRNA 表达水平与 SLEDAI 呈正相关(P<0.01);抗 dsDNA 抗体联合IL-21、BCL6、CD71 mRNA表达诊断SLE的AUC为0.952,敏感度和特异度分别为92.7%和87.8%.结论:SLE患者IL-21、BCL6、CD71 mRNA表达水平显著升高,且与SLEDAI呈正相关;抗dsDNA抗体联合IL-21、BCL6、CD71 mRNA检测可显著提高早期SLE的诊断效能.
Objective To investigate the clinical diagnostic value of anti-double-stranded(ds)DNA antibody combined with interleukin-21(IL-21),B-cell lymphoma 6(BCL6),and transferrin receptor(CD71)mRNA levels for early sys-temic lupus erythematosus(SLE).Methods Blood samples were collected from 41 newly diagnosed SLE patients(inclu-ding 8 in the active phase and 33 in the inactive phase)and 41 healthy controls between January 2021 and June 2025.Pe-ripheral blood mononuclear cells(PBMCs)were isolated,and the mRNA levels of IL-21,BCL6,and CD71 were meas-ured.Serum anti-dsDNA antibodies were detected by immunoblotting.Spearman correlation analysis was used to assess the relationship between the expression of the markers and the SLE disease activity index(SLEDAI)scores.Receiver operating characteristic(ROC)curve analysis was performed to evaluate the diagnostic efficacy of anti-dsDNA antibody combined with IL-21,BCL6,and CD71 gene expression for SLE.Results The levels of IL-21,BCL6,and CD71 in PBMCs were higher in SLE patients than in healthy controls(P<0.05),and their expressions were elevated in active SLE patients com-pared to inactive patients(P<0.0001).Spearman correlation analysis indicated that the expressions of these markers were positively correlated with SLEDAI scores(P<0.01).The combination of anti-dsDNA antibodies and IL-21,BCL6,and CD71 genes achieved an area under the curve(AUC)of 0.952,with a sensitivity of 92.7%and specificity of 87.8%.Con-clusion The expressions of IL-21,BCL6,and CD71 genes are significantly elevated in SLE patients and positively corre-late with SLEDAI scores.Anti-dsDNA antibodies along with IL-21,BCL6,and CD71 gene expressions provide excellent diagnostic performance for early SLE.
