健脾益肾祛瘀法联合穴位贴敷对糖尿病肾病(G3A2期)合并骨质疏松症的临床疗效OA
Clinical Effect of Jianpi Yishen Quyu Method Combined with Acupoint Application on Diabetic Nephropathy(G3A2 Stage)Complicated with Osteoporosis
目的 分析健脾益肾祛瘀法联合穴位贴敷在糖尿病肾病(G3A2 期)合并骨质疏松症患者中的临床疗效.方法 选取 2024年1 月-10 月九江市中医医院就诊的 94 例糖尿病肾病(G3A2 期)合并骨质疏松症患者作为研究对象,按照随机数字表法将其分为对照组和治疗组,每组 47 例.两组均实施常规基础治疗,对照组在常规基础治疗基础上使用碳酸钙D3 片及骨化三醇软胶囊治疗,治疗组在常规基础治疗基础上加用健脾益肾祛瘀法联合穴位贴敷治疗.比较两组临床疗效、血清指标水平[血钙、血磷、碱性磷酸酶(ALP)、25-羟基维生素D]、骨密度[腰椎(L1～L4)、股骨颈骨]、中医证候积分、不良反应(低血糖、腹泻、头疼、皮疹)发生率.结果 与对照组治疗总有效率(76.60%)相比,治疗组更高,达 95.74%(P＜0.05);治疗后,治疗组血清钙、25-羟基维生素D水平及股骨颈骨、腰椎(L1～L4)骨密度均高于对照组,而血清磷、ALP水平均低于对照组(P＜0.05);治疗组治疗后中医证候积分(主症、次症)均低于对照组(P＜0.05);与对照组不良反应发生率(17.02%)相比,治疗组更低,仅为 4.26%(P＜0.05).结论 健脾益肾祛瘀法联合穴位贴敷治疗糖尿病肾病(G3A2 期)合并骨质疏松症患者的临床疗效确切,能够提高患者血清钙、25-羟基维生素D水平及骨密度,降低患者血清磷、ALP水平,减轻患者临床症状,且安全性较高.
Objective To analyze the clinical efficacy of Jianpi Yishen Quyu method combined with acupoint application in patients with diabetic nephropathy(G3A2 stage)complicated with osteoporosis.Methods A total of 94 patients with diabetic kidney disease(stage G3A2)complicated by osteoporosis,who were treated at Jiujiang Hospital of Traditional Chinese Medicine from January to October 2024,were selected as the study subjects.They were divided into a control group and a treatment group using a random number table method,with 47 patients in each group.Both groups received conventional basic treatment.On this basis,the control group was treated with calcium carbonate and vitamin D3 tablets combined with calcitriol soft capsules,while the treatment group was additionally treated with Jianpi Yishen Quyu method and acupoint application.The clinical efficacy,serum indicator levels[serum calcium,serum phosphorus,alkaline phosphatase(ALP),25-hydroxyvitamin D],bone mineral density[lumbar spine(L1~L4),femoral neck],traditional Chinese medicine syndrome scores,and the incidence of adverse reactions(hypoglycemia,diarrhea,headache,rash)were compared between the two groups.Results Compared with the total effective rate of the control group(76.60%),the treatment group was higher,reaching 95.74%(P＜0.05).After treatment,the levels of serum calcium,25-hydroxyvitamin D and bone mineral density of femoral neck and lumbar spine(L1-L4)in the treatment group were higher than those in the control group,while the levels of serum phosphorus and ALP were lower than those in the control group(P＜0.05).After treatment,the scores of traditional Chinese medicine syndrome(main symptom and secondary symptom)in the treatment group were lower than those in the control group(P＜0.05).Compared with the control group(17.02%),the incidence of adverse reactions in the treatment group was lower,only 4.26%(P＜0.05).Conclusion Jianpi Yishen Quyu method combined with acupoint application has a definite clinical effect in the treatment of patients with diabetic nephropathy(G3A2 stage)complicated with osteoporosis,which can improve the serum calcium,25-hydroxyvitamin D level and bone mineral density,reduce the serum phosphorus and ALP levels,and relieve the clinical symptoms of patients,with high safety.
