数字X线摄影和CT影像检查在脊柱爆裂性骨折中的诊断价值OA
Diagnostic Value of Digital Radiography and CT Imaging in Burst Fractures of the Spine
目的 探讨数字X线摄影(DR)和CT影像检查在脊柱爆裂性骨折中的诊断价值.方法 选取 2023 年 1 月-2024 年 11 月万年县人民医院收治的疑似脊柱爆裂性骨折的 60 例患者,采用随机数字表法将其分为对照组和研究组,每组 30 例.对照组选择DR诊断方法,研究组选择CT影像检查.对两组诊断效果进行统计评估,并探讨不同检查方式下脊柱爆裂性骨折患者的检查时间和检查费用、检出率、诊断效能以及解剖学符合率等情况.结果 研究组检查时间以及检查费用分别为(8.39±0.48)min、(601.23±11.35)元,均高于对照组的(2.39±0.43)min、(117.32±10.38)元(P<0.05).研究组检出率为 96.67%,高于对照组的76.67%(P<0.05).研究组诊断准确率、特异度、敏感度分别为 93.33%、96.67%、83.33%,均高于对照组的 66.67%、76.67%、33.33%;而研究组误诊率、漏诊率分别为 3.33%、13.33%,均低于对照组的 20.00%、63.33%(P<0.05).研究组诊断脊柱爆裂性骨折和解剖学符合率分别达到 83.33%、93.33%、76.67%、83.33%、83.33%,均高于对照组的 30.00%、56.67%、33.33%、33.33%、40.00%(P<0.05).结论 为脊柱爆裂性骨折患者提供诊断工作时,采用CT影像检查方式,可以更加了解患者的脊柱骨折情况,提升检出率和准确率,减少误诊和漏诊的情况.但该检查方式检查费用高和检查时间比较长,所以应根据患者的实际情况进行合理应用.
Objective To investigate the diagnostic value of digital radiography(DR)and CT imaging in burst fractures of the spine.Methods A total of 60 patients with suspected spinal burst fracture admitted to Wannian County People's Hospital from January 2023 to November 2024 were selected and divided into control group and study group by random number table method,with 30 patients in each group.The control group selected digital radiography(DR)diagnosis method,and the study group selected CT imaging examination.The diagnostic effects of the two groups were statistically evaluated,and the examination time,examination cost,detection rate,diagnostic efficiency and anatomical coincidence rate of patients with spinal burst fracture under different examination methods were discussed.Results The examination time and cost of the study group were(8.39±0.48)min and(601.23±11.35)yuan,respectively,which were higher than(2.39±0.43)min and(117.32±10.38)yuan of the control group(P<0.05).The detection rate of the study group was 96.67%,which was higher than 76.67%of the control group(P<0.05).The diagnostic accuracy,specificity and sensitivity of the study group were 93.33%,96.67%and 83.33%,respectively,which were higher than 66.67%,76.67%and 33.33%of the control group;the misdiagnosis rate and missed diagnosis rate of the study group were 3.33%and 13.33%,respectively,which were lower than 20.00%and 63.33%of the control group(P<0.05).The coincidence rates of diagnosis of spinal burst fracture and anatomy in the study group were 83.33%,93.33%,76.67%,83.33%and 83.33%,respectively,which were higher than 30.00%,56.67%,33.33%,33.33%,40.00%in the control group(P<0.05).Conclusion CT imaging provides superior visualization of burst fractures of the spine,enhancing detection and diagnostic accuracy,and reducing misdiagnosis and missed diagnosis.However,the higher costs and longer examination times necessitate careful consideration based on individual patient needs.
